腹腔镜下腹股沟淋巴清扫术应用于外阴癌手术治疗中的疗效

Curative Effect of Laparoscopic Inguinal Lymph Dissection in the Treatment of Vulvar Carcinoma

目的:探讨腹腔镜下腹股沟淋巴清扫术(LIND)应用于外阴癌手术治疗中的疗效。方法:选取2012年7月~2017年3月间某院收治的36例外阴癌患者为研究对象,征求患者意见后,应用计算机随机分配软件分为两组。对照组19例行传统开放LIND治疗,研究组17例行腹腔镜下LIND治疗,对比两组相关手术指标及相关并发症发生情况。结果:两组切除外阴耗时、外阴整形重建耗时、外阴手术失血量以及双侧腹股沟淋巴切除数量等方面相比,差异无统计学意义(P>0.05);研究组手术全程耗时,LIND操作耗时及清扫出血量,总出血量与对照组相比,差异有统计学意义(P<0.05);研究组患者术后住院时间短于对照组,差异有统计学意义(P<0.05);研究组患者术后腹股沟皮肤的并发症及淋巴相关并发症的发生率显著低于对照组,差异有统计学意义(P<0.05);有关外因伤口并发症的发生率,两组比较接近,差异无统计学意义(P>0.05)。结论:腹腔镜下腹股沟淋巴清扫术应用于外阴癌手术治疗,可有效降低相关并发症的发生率,促进患者快速恢复,值得推广。

Objective：To investigate the effect of laparoscopic inguinal lymph dissection in the treatment of vulvar carcinoma.Methods：36 cases of vulvar cancer patients treated in a hospital from July 2012 to March2017 were selected as the research objects and randomly divided into control group（19 cases）and study group（17 cases）by computer software.The control group was given conventional open LIND treatment,and the study group was given laparoscopic LIND treatment,then the related indicators and complications were compared between the two groups.Results：There was no significant difference in time consuming for vulva excision,the time consuming for reconstruction of vulva,the amount of blood loss in the vulvar operation,and the number of bilateral inguinal lymphadenectomy between the two groups（P〈0.05）.Compared with the control group,the duration of operation,LIND operation time consuming,cleaning bleeding volume,and the total blood loss in the study group were significantly different from those in the control group（P〈0.05）.The postoperative hospitalization time of the patients in the study group was shorter than that in the control group,and the difference was statistically significant（P〈0.05）.The incidence of postoperative inguinal skin complications and lymphoid complications in the study group was significantly lower than that in the control group,the difference was statistically significant（P〈0.05）.The incidence of external wound complications was similar between the two groups,and the difference was not statistically significant（P〈0.05）.Conclusion：Laparoscopic inguinal lymph dissection in the treatment of vulvar carcinoma can effectively reduce the incidence of related complications and promote patients recover quickly,which is worthy of promotion.
