摘要
目的:探讨院内急性胸痛绿色通道在急性ST段抬高型心肌梗死(STEMI)患者救治中的应用效果及价值。方法:将某院于2015年11月~2016年4月采用传统流程救治的78例STEMI患者,同2016年5月~2016年10月采用院内急性胸痛绿色通道救治的98例STEMI患者实施救治后的临床数据进行对比分析。结果:在观察组98例患者中,急诊冠脉造影诊断冠状动脉正常的患者为8例,行PCI者90例,平均D2B为(69.63±6.94)min,其中有88.73%达到胸痛中心要求的<90min;在对照组78例患者中,造影诊断正常的患者为6例,行PCI者72例,平均D2B为(112.51±7.85)min,观察组入院至球囊扩张时间与平均住院天数均小于对照组,院内病死率同对照组相比差异显著,两者均具有统计学意义(P<0.05)。结论:实施创建胸痛中心以来,我院采取院内急性胸痛绿色通道大大减少了对STEMI患者救治时间的延误,并缩短患者的住院时间,减少病死率,提高了患者的生活水平和生存质量,值得在本领域大力推广。
Objective:To investigate the effect and value of green channel in the treatment of acute STsegment elevation myocardial infarction(STEMI).Methods:The clinical data of 78 patients with STEMI treated with traditional procedure from November 2015 to April 2016 were compared with 98 STEMI patients treated with green channel in hospital from May 2016 to October 2016.The clinical data after treatment were compared and analyzed between two groups.Results:Among the 98 patients in the observation group,8 cases were diagnosed as normal by emergency coronary angiography,90 cases were treated with PCI,the average D2 Bwas(69.63±6.94)min,and 88.73% of them met the requirement of chest pain center(requirement90 min);Among the 78 patients in the control group,6 cases were diagnosed as normal by angiography,72 cases were treated with PCI,the average D2 Bwas(112.51±7.85)min.The time from admission to balloon dilatation and the average length of hospitalization in the observation group were smaller than those in the control group,and the mortality in hospital was significantly different from that in the control group,the difference between the two groups was statistically significant(P〈0.05).Conclusion:Since the establishment of chest pain center,the green channel of acute chest pain in a hospital has greatly reduced the delay in the treatment of STEMI patients,shortened the hospitalization time,reduced the mortality rate,and improved the living standard and quality of life of the patients,which is worth popularizing in this field.
