重组组织型纤溶酶原激活剂联合中成药治疗缺血性脑卒中的荟萃分析

Recombinant Tissue Type Plasminogen Activator and Traditional Chinese Medicine in the Treatment of Ischemic Stroke:a Meta Analysis

目的通过Meta荟萃分析,探讨重组组织型纤溶酶原激活剂(rt-PA)联合中成药治疗缺血性脑卒中的价值。方法检索2010年1月至今中英文相关期刊数据库,收集rt-PA、rt-PA联合中成药治疗急性缺血性脑卒中的随机或非随机对照试验,按照纳入与排除标准选择、提取资料和评价质量后进行Meta荟萃分析。结果纳入7项研究,Meta分析结果显示,rt-PA联合中成药组治疗急性缺血性脑卒中的(3~6)个月神经功能状况高于单用rt-PA[RR=1.22,95%CI(1.10,1.35),P=0.00];rt-PA联合中成药组的术后并发症低于rt-PA组[RR=3.94,95%CI(2.31,6.70),P=0.00],但两组的(3~6)个月总体死亡率无统计学意义。结论重组组织型纤溶酶原激活剂联合中成药治疗缺血性脑卒中能促进改善神经功能,减少术后并发症的发生,但是不延长病人的生存时间。

Objective To evaluate the values of recombinant tissue type plasminogen activator （rt PA） combined with traditional Chi nese medicine in the treatment of ischemic stroke by the meta analysis.Methods From January 2010 up to now,the English and Chi nese journals database were given the retrieval,the randomized or non randomized controlled trials treated by the rt PA, rt PA and traditional Chinese medicine in the treatment of acute ischemic stroke were selected,according to the inclusion and exclusion criteria, for meta analysis.Results There were seven literatures were included,the meta analysis results show that the neurological status af ter treatment 3 6 months by the rt PA and traditional Chinese medicine were higher than that of rt PA [RR= 1.22,95% Cl （1.10,1.35）, P = 0.00];and the postoperative complications were lower than the rt PA group [RR = 3.94,95% CI （2.31,6.70）, P = 0.00].There was no difference in overall mortality of 3 6 months between two groups.Conclusion The rt PA and traditional Chinese medicine can promote the improvement of neurological function,reduce the incidence of postoperative complications,but do not prolong the survival time in patients with ischemic stroke.
