整体护理对甲状腺癌患者术后康复和心理状况的影响

THE EFFECT OF HOLISTIC NURSING ON POSTOPERATIVE REHABILITATION AND PSYCHOLOGICAL STATUS OF PATIENTS WITH THYROID CANCER

目的分析整体护理对甲状腺癌根治手术后患者康复和心理健康状况的影响。方法选取2014年3月~2016年12月间我院收治甲状腺癌并行根治手术患者84例作为研究对象,随机分为观察组和对照组各42例,其中对照组给予外科常规护理,观察组给予整体护理干预模式,术后随访6个月。采用焦虑自评量表(SAS)和抑郁自评量表(SDS)评估患者心理健康状况,以癌症患者核心量表(QLC-C30)分析患者生存质量,分别在入组时、术后3个月、6个月时进行问卷调查,同时记录术后6个月内并发症发生情况。结果入组时两组患者间SAS、SDS以及QLC-C30评分无统计学差异(P>0.05);术后3个月时,观察组SAS和SDS评分低于对照组,差异具有统计学意义(P<0.05),QLC-C30评分高于对照组,但差异无统计学差异(P>0.05);术后6个月时,观察组QLC-C30评分中生理功能、角色智能、情绪功能、社会功能以及总体生活质量方面评分均高于对照组,差异具有统计学意义(P<0.05)。并发症方面,观察组共发生3例(低钙血症2例,呼吸困难1例),占7.14%;对照组发生10例,占23.80%,高于观察组,差异具有统计学意义(P<0.05)。结论整体护理干预有助于改善甲状腺癌术后患者的心理健康状态,减少术后并发症的发生,提高生活质量,在临床上有一定应用价值。

Objective To analyze the effect of holistic nursing on postoperative rehabilitation and mental status of patients with thyroid cancer.Methods84 cases of thyroid cancer having received radical surgery in our hospital during March,2014 to December,2016 were randomly and evenly divided into the observation group and the control group.The control group patients were given surgical routine nursing,while the observation group patients received holistic nursing,with the postoperative follow-up surveys lasting 6 months.Self-rating Anxiety Scale（SAS）and Self-Rating Depression Scale（SDS）were used to evaluate the mental status,and QLC-C30 was used to analyze patients＇ quality of life.The questionnaire surveys were conducted at the beginning of grouping,3 months and 6 months after operations respectively,and the postoperative complications within 6 month were also recorded.Results There were no statistical difference in SAS,SDS and QLC-C30 scores at the beginning of grouping between the two groups（P 0.05）.Three months later,the SAS and SDS scores in the observation group were lower than those of the control group（P 0.05）,but the QLC-C30 score in the ob-servation group was higher than that in the control group（P 0.05）.Six months later,patients＇ physiological functions,the intelligence in role-playing,the emotional functions,social functions and the overall quality of life in observation group showed higher scores than those in the control group（P 0.05）.Three cases of complications（7.14%,including 2 cases of hypocalcemia and one case of dyspnea）occurred in the observation group,while 10 cases of complications（23.80%）occurred in the control group（P 0.05）.Conclusion Holistic nursing can help to improve the postoperative mental status,reduce postoperative complications and improve the postoperative quality of life of the patients with thyroid cancer.
