多学科护理模式对喉癌术后下呼吸道感染患者自护能力与生活质量的影响

Effect of multi-disciplinary nursing model on self-care ability and quality of life in patients with lower respiratory tract infection after laryngeal cancer surgery

目的探讨多学科护理模式对喉癌术后下呼吸道感染患者自护能力与生活质量的影响。方法选取2014年1月至2017年2月我院收治的喉癌术后下呼吸道感染患者48例,将其随机等分为对照组和研究组,对照组患者实施常规护理干预,研究组实施多学科护理模式干预,比较护理干预前后两组患者焦虑自评量表(SAS)、抑郁自评量表(SDS)、自我护理能力测定量表(ESCA)、WHO生存质量测定量表汉化版(WHO QOL-BREF)评分及不良事件发生率。结果多学科干预后,研究组患者SAS,SDS评分低于对照组(P<0.05);健康知识水平、自护责任感、自我护理技能、自我概念评分高于对照组(P<0.05);多学科干预后1,3,5个月WHO QOL-BREF评分高于对照组(P<0.05);不良事件发生率低于对照组(P<0.05)。结论多学科护理模式应用在喉癌术后下呼吸道感染患者中,可有效减轻患者焦虑、抑郁情绪、提高其自护能力与生活质量,减少术后不良事件发生,值得临床推广应用。

Objective To explore the effect of multi-disciplinary nursing model on self-care ability and quality of life in patients with lower respiratory tract infection after laryngeal cancer surgery. Methods To select patients of 48 cases with postoperative lower respiratory tract infection treated in our hospital from January 2014 to February 2017,and they were randomly divided into a control group and a study group. The control group was received routine nursing interventions,and the study group was performed with multiple disciplines. Nursing model interventions were used to compare the scores of self-rating anxiety scale（SAS）,self-rating depression scale（SDS）,exercise of self-care agency scale（ESCA）,and the Chinese version of WHO quality of life scales（WHO QOL-BREF） as well as the incidence of adverse events between the two groups before and after nursing intervention. Results After multidisciplinary intervention,the SAS and SDS scores of the study group were lower than that of the control group（P 〈 0. 05）. The level of health knowledge,self-care responsibility,self-care skills,and self-concept score were higher than that of the control group（P 〈 0. 05）. After 1,3,and 5 months of multidisciplinary intervention,the WHO QOL-BREF score was higher than that of the control group（P 〈 0. 05） and the incidence of adverse events was lower than that of the control group（P 〈 0. 05）. Conclusion The application of multidisciplinary nursing model in patients with postoperative lower respiratory tract infection of laryngeal cancer can effectively relieve anxiety and depression. Meanwhile,it can improve self-care ability and quality of life,and reduce postoperative adverse events. Therefore,it is worthy of clinical application.
