摘要
目的探讨高血压脑出血患者单核细胞/淋巴细胞比率及其临床意义。方法选择高血压脑出血患者70例为研究组,健康志愿者60例为对照组。比较两组的MLR、中性粒细胞/淋巴细胞比率(NLR)和血小板/淋巴细胞比率(PLR)。应用受试者工作特征曲线分析MLR、NLR和PLR对高血压脑出血的诊断价值。根据MLR的临界值将高血压脑出血患者分为MLR低值组和MLR高值组,比较两组的临床资料、住院及随访期间死亡率。结果研究组的MLR、NLR及PLR均高于对照组(P<0.05)。MLR、NLR和PLR诊断高血压脑出血的曲线下面积分别为0.792、0.675和0.750,其中MLR的曲线下面积均大于NLR、PLR(均P<0.05);临界值为0.258时,MLR的敏感性为70.3%,特异性为75.5%。与MLR低值组相比,MLR高值组年龄、贫血率、吸烟率、随访期间死亡者比例均更高(均P<0.05)。结论高血压脑出血患者的MLR升高;MLR可能与高血压出血患者病情进展甚至死亡关系密切,具有较高的诊断价值。
Objective To explore the monocyte-to-lymphocyte ratio( MLR) in hypertensive cerebral hemorrhage and its clinical significance. Methods Seventy patients with hypertensive cerebral hemorrhage were enrolled as study group,and 60 healthy volunteers as control group. The MLR,neutrophil-to-lymphocyte ratio( NLR) and platelet-to-lymphocyte ratio( PLR) were compared between the two group. Receiver operating characteristic curve was used to analyze the diagnostic value of MLR,NLR and PLR for hypertensive cerebral hemorrhage. The patients with hypertensive cerebral hemorrhage were divided into low-MLR group and high-MLR group according to the cutoff value of MLR. The clinical data and mortalities during hospitalization and follow-up were compared between the two groups.Results MLR,NLR and PLR were higher in the study group than to those in the control group( P〈0. 05). For diagnosing hypertensive cerebral hemorrhage,the areas under the curve of MLR,NLR and PLR were 0. 792,0. 675 and 0. 750 respectively,and the areas under the curve of MLR was larger than that of NLR or PLR( all P〈0. 05). The cutoff value of MLR was 0. 258 with sensitivity of 70. 3% and specificity of75. 5%. Compared to the low-MLR group,the high-MLR group had higher proportions of older cases,anemic cases and smokers as well as higher mortality during follow-up( all P〈0. 05). Conclusion MLR is elevated in patients with hypertensive cerebral hemorrhage,is probably related to the progression even death of hypertensive cerebral hemorrhage,and obtains high diagnostic value.
