MRI检查对膝关节损伤的诊断价值

Diagnostic value of MRI for knee injury

目的探讨MRI检查诊断膝关节损伤的价值。方法选取疑似膝关节损伤患者300例,分别行MRI检查和关节镜检查。分析两种检查方法的一致性。结果关节镜检查确诊关节腔积液73例,半月板损伤62例,韧带损伤45例,骨质改变40例,无关节损伤者80例。MRI检查诊断关节腔积液73例,半月板损伤62例,韧带损伤45例,骨质改变42例,无关节损伤者78例。MRI检查诊断膝关节损伤的敏感性为100.0%,特异性为97.50%,阳性预测值为100.0%,阴性预测值为97.50%。MRI和关节镜检查对膝关节损伤的检出率比较,差异无统计学意义(P>0.05)。结论 MRI检查诊断膝关节损伤有较高的临床价值。

Objective To investigate the value of MRI in the diagnosis of knee injury. Methods Totally 300 patients with suspected knee injury received knee arthroscopy and MRI respectively. And the consistency of the two examination methods was analyzed.Results Arthroedema confirmed by arthroscopy were found in 73 cases,meniscus injury in 62 cases,ligament injury in 45 cases,bone changes in 40 cases,and no knee injury in 80 cases. Arthroedema confirmed by MRI were found in 73 cases,meniscus injury in 62 cases,ligament injury in 45 cases,bone changes in 42 cases,and no knee injury in 78 cases. The sensitivity,specificity,positive predictive value and negative predictive value of MRI for diagnosing knee injury were 100. 0%,97. 50%,100. 0% and 97. 50% respectively. There was no statistically significant difference in the detection rate of knee injury between MRI and arthroscopy（ P〉 0. 05）. Conclusion MRI has a higher clinical value in the diagnosis of knee injury.
