经阴道超声诊断宫腔粘连的临床应用价值

Clinical application value of transvaginal ultrasound in diagnosis of intrauterine adhesions

目的探究宫腔粘连应用阴道超声诊断的临床效果。方法选择我院及广东省妇幼保健2015年1月-2018年2月收治的92例高度疑似宫腔粘连患者,所有患者入院后均接受经阴道超声检查,将超声诊断结果与宫腔镜检查结果进行对比,分析阴道超声检查结果的准确性情况。结果经宫腔镜检查有宫腔粘连82例,发生率为89.13%;经阴道超声检查,92例患者中宫腔粘连79例,检出率为85.87%,经阴道超声诊断阳性检出率与宫腔无明显差异(P>0.05);经对比分析经阴道超声检查确诊79例,准确率为96.34%,漏诊3例,漏诊率为3.26%。结论经阴道超声检查在宫腔粘连诊断中具有较高的准确性,可较好观察宫腔内组织及子宫输卵管病变情况,准确评估子宫内膜受损情况,且操作无创、安全,为宫腔粘连诊断提供可靠的依据。

Objective To explore the clinical effect of vaginal ultrasound diagnosis of intrauterine adhesions. Methods A total of 92 patients with highly suspected intrauterine adhesions who were admitted to our hospital and Guangdong Maternal and Child Health Hospital from January 2015 to February 2018 were enrolled. All patients underwent transvaginal ultrasound examination after admission. Ultrasound diagnosis and hysteroscopy were performed. The results of the examination were compared to analyze the accuracy of the results of vaginal ultrasound examination. Results There were 82 cases of intrauterine adhesions by hysteroscopy, the incidence rate was 89.13%. Transvaginal ultrasonography, 79 cases of intrauterine adhesions in 92 cases, the detection rate was 85.87%, positive examination by transvaginal ultrasound diagnosis. There was no significant difference between the rate and the uterine cavity（P〈0.05）. Seventy-seven patients were diagnosed by vaginal ultrasonography, the accuracy rate was 96.34%, 3 cases were missed, and the missed diagnosis rate was 3.26%. Conclusion Transvaginal ultrasonography has high accuracy in the diagnosis of intrauterine adhesions. It could better observe the intrauterine tissue and uterine fallopian tube lesions, accurately assess the endometrial damage, and the operation is non-invasive and safe. The diagnosis of intrauterine adhesions provides a reliable basis.
