伽玛刀治疗单发小脑转移瘤的临床疗效分析

Clinical Analysis of Gamma knife in the treatment of single cerebellar Metastasis

目的本研究旨在探讨伽玛刀治疗单发小脑转移瘤的临床疗效,并促进越来越多的文献证明立体定向放射外科(SRS)治疗小脑转移的安全性和有效性。方法回顾性分析2016年6月~2017年8月35例单发小脑转移患者接受伽玛刀治疗的临床资料,评估临床症状及肿瘤控制情况。结果本组35例患者伽玛刀术后临床症状均获不同程度缓解。随访1~19个月(平均8.09±4.15个月),随访截止时13例存活(37%),22例死亡(63%),其中8例(36.4%)死因为颅内病灶进展,14例(63.6%)为原灶进展及多脏器衰竭。患者中位生存期为:11个月,6个月生存率达65.7%。结论伽玛刀治疗小脑转移瘤安全、有效,短期内能明显改善患者生存质量,有益于延长生存期,减少并发症,为后期综合治疗提供条件。

Objective The purpose of this study was to investigate the clinical efficacy of gamma knife in the treatment of single cerebellar metastases and to promote more and more literatures to prove the safety and efficacy of stereotactic radiosurgery（SRS）in the treatment of cerebellar metastases.Methods The clinical data of 35 patients with single cerebellar metastasis received gamma knife therapy from June 2016 to August 2017 were analyzed retrospectively to evaluate the clinical symptoms and tumor control.Results The clinical symptoms of 35 patients were relieved in different degree after gamma knife operation.Follow-up for 1 to 19 months（mean 8.09±4.15 months）,13 cases survived and 22 cases died at the end of follow-up,of which 8 cases（36.4%）died because of intracranial lesion progression,14 cases（63.6%）were primary lesion progression and multiple organ failure.The median survival time was 11 months and the 6 month survival rate was 65.7%.Conclusion Gamma knife is safe and effective in the treatment of cerebellar metastases.In the short term,it can obviously improve the quality of life of the patients.It is beneficial to prolong the survival time,reduce the complications and provide conditions for comprehensive treatment in the later stage.
