1例重症急性胰腺炎患者营养支持治疗的临床药学实践

Pharmaceutical care for a patient with severe acute pancreatitis with nutrition support

临床药师参与1例重症胰腺炎患者临床营养支持治疗过程,并根据病情变化对该患者营养支持方案进行制订与调整,对患者实施药学监护。在这个过程中结合患者病情调整肠外营养组方、更换肠内营养制剂、停用肠外营养、监测血糖及电解质等措施,提高了营养支持药物的合理使用,保证了患者的用药安全,使患者的症状得到了较好的改善。通过此案例显示了临床药师参与营养支持治疗的必要性,通过协助医师制订更加安全有效的个体化营养治疗方案,并加强药学监护,确保患者营养支持的安全、有效性。

Clinical pharmacists participated in the clinical nutrition support treatment for one patient with severe acute pancreatitis. According to the patient＇s condition, a nutrition support program was formulated and adjusted, and the patient was subjected to pharmaceutical care. In this process, based on the patient＇s condition, adjustment of parenteral nutrition prescriptions, replacement of enteral nutrition drug, withdrawal of parenteral nutrition, monitoring of blood glucose and electrolytes and other measures were conducted to improve the rational use of nutritional drugs to ensure the patient＇s medication safety, and the patient＇s symptoms were improved. This case showed the importance of clinical pharmacists in nutrition support treatment to ensure safer and more effective individualized nutrition programs.
