临床药师充分利用药学专业知识,以药物的不良反应、适应证与相互作用为切入点,对1例消化道出血患者的用药史进行详尽调查和用药评估,通过临床思维与经验分析归纳,发现其发病原因,优化临床抗凝药物治疗方案,停用了作用重叠、与适应证不符的药物,患者充分得到了个体化治疗,也显示了合理用药的重要性。

Clinical pharmacists make full use of pharmaceutical knowledge to investigate and elucidate the medication history of gastrointestinal bleeding in one patient. Based on clinical thinking and experience, we used the drug adverse reactions, indications and interaction as the breakthrough point to find the etiology of bleeding, optimized the anticoagulation project and stopped unnecessary drugs. The patient received fully individualized treatment and recovered soon.
