自拟愈疡汤保留灌肠联合美沙拉嗪口服治疗溃疡性结肠炎的疗效

Efficacy of retention enema with self-made ulcer-healing decoction plus oral mesalazine for ulcerative colitis

目的探讨自拟愈疡汤保留灌肠联合美沙拉嗪口服治疗溃疡性结肠炎的疗效。方法选取河南省某医院2015年1月-2017年1月收治的84例溃疡性结肠炎患者为研究对象,采用随机数字表法分为对照组(n=41)与观察组(n=43);对照组患者给予单纯西药治疗,观察组患者给予自拟愈疡汤保留灌肠联合美沙拉嗪口服治疗;比较2组患者临床疗效、不良反应发生情况及治疗前后的炎性因子水平、中医证候积分。结果治疗前,2组患者白细胞介素-8(IL-8),白细胞介素-10(IL-10)及肿瘤坏死因子-α(TNF-α)水平比较,差异无统计学意义(P>0.05);治疗后,2组患者IL-8,TNF-α水平低于治疗前,IL-10水平高于治疗前,且观察组患者IL-8,TNF-α水平低于对照组,IL-10水平高于对照组,差异均有统计学意义(P<0.05)。观察组患者治疗总有效率高于对照组,差异有统计学意义(P<0.05)。治疗前,2组患者腹痛、腹泻、脓血便中医证候积分比较,差异无统计学意义(P>0.05);治疗后,2组患者腹痛、腹泻、脓血便中医证候积分低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。2组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论自拟愈疡汤保留灌肠联合美沙拉嗪口服治疗溃疡性结肠炎,可显著改善患者症状,减轻炎症反应且安全性较好,值得临床推广使用。

Objective To assess the efficacy of retention enema with self-made ulcer-healing decoction plus oral mesalazine for ulcerative colitis.Methods A total of 84 patients with ulcerative colitis admitted into a Henan hospital between January 2015 and January 2017 were included in this study.Based on a random number table,patients were divided into the control group（n=41）or the trial group（n=43）.Patients in the control group received conventional treatment with western medicine,while those in the trial group received retention enema with self-made ulcer-healing decoction plus oral mesalazine.The clinical response,adverse reactions,inflammatory factor levels and TCM syndrome scores before and after treatment were compared between the two groups.Results The pre-treatment IL-8,IL-10,and TNF-αlevels were similar between the two groups（P〉0.05）.After treatment,the IL-8 and TNF-αlevels were decreased and the IL-10 level was increased in both groups when compared with their pre-treatment levels;the IL-8 and TNF-αlevels in the trial group were significantly lower than those in the control group,while the IL-10 level was significantly higher than that in the control group（P〈0.05）.The overall response rate in the trial group was significantly higher than that in the control group（P〈0.05）.The pre-treatment TCM syndrome scores of abdominal pain,diarrhea,and bloody purulent stool were comparable between the two groups（P〉 0.05）.After treatment,the TCM syndrome scores of abdominal pain,diarrhea,and bloody purulent stool were reduced in both groups when compared with their pre-treatment scores;and these scores in the trial group were significantly lower than those in the control group（P〈0.05）.The incidence of adverse reactions were similar between the two groups（P〉0.05）.Conclusion For patients with ulcerative colitis,retention enema with self-made ulcer-healing decoction plus oral mesalazine is safe and effective in improving clinical symptoms and relieving inflammatory response,which deserves widespread clinical application.
