探讨光学内镜与染色内镜在食管早癌及癌前病变中的临床应用价值

Clinical value analysis of optical endoscopy and chromoendoscopy in diagnosis of early esophageal carcinoma and precancerous lesions

[目的]比较普通白光内镜(white light imaging WLI)、光学内镜(内镜窄带成像技术Narrow Band Imaging NBI)与染色内镜(Lugol’s碘液染色)在食管早癌及癌前病变中的临床应用价值。[方法]选取2016年1月~2017年12月我院消化内镜中心门诊疑似食管黏膜病变患者81例,顺次在普通白光内镜、NBI和Lugol’s碘染观察后,记录病变的部位、大小、数量、范围、黏膜形态等,并进行NBI及Lugol’s碘染分级,对所有阳性病灶进行病理组织学取材。将所有数据记录结果进行统计分析。[结果]光学内镜和染色内镜均能更加清晰显示病灶范围及大小,提高活检准确性及病变检出率,且对于食管早癌和癌前病变的诊断效能相似,均能优于单纯普通白光内镜检查。[结论]本文探讨光学内镜与染色内镜对食管早癌及癌前病变的诊断价值,值得临床推广应用。

[Objective]To compare the clinical application value of White light imaging（WLI）,optical endoscopy（narrow-band imaging NBI）and chromoendoscopy（Lugol＇s iodine staining）in diagnosis of early esophageal carcinoma and precancerous lesions.[Methods]81 cases of patients were enrolled in our study,who were suspected of being esphageal mucosal lesions from our hospital from January 2016 to December2017.After observing with white light endoscope,NBI and Lugol＇s iodine staining,the site,size,number,range,mucosal morphology,etc.of lesions were recorded,and NBI and Lugol＇s iodine staining were performed.All positive lesions were obtained by pathological histology.[Results]NBI and Lugol＇s iodine staining can show the scope and size of early esophageal carcinoma and precancerous lesions more clearly,improving lesion detection rate and accuracy of biopsy.The optical endoscopy has the same effect as chromoendoscopy,and they are significantly better than WLI.[Conclusion]This article discusses the value of optical endoscopy and chromoendoscopy in the diagnosis of early esophageal carcinoma and precancerous lesions,and is worthy of clinical application.
