纳布啡预防全麻苏醒期躁动的临床观察

Clinical observation of naborphine in preventing emergence agitation of general anesthesia

目的探究纳布啡对腹腔镜下保胆取石患者复苏期的影响。方法选取天津百信医院2015年12月-2016年12月腹腔镜下保胆取石术患者72例,利用随机数字表法分为观察组和对照组各36例。两组患者均采用常规麻醉,手术结束前30 min,观察组静脉注射0.2 mg/kg的盐酸纳布啡注射液,对照组注射同等剂量的生理盐水。比较两组患者的苏醒时间和拔管时间、苏醒后的躁动程度、不良反应。结果观察组与对照组相比苏醒时间和拔管时间差异无统计学意义(P>0.05);观察组的躁动程度评分低于对照组,差异有统计学意义(P<0.05)。对照组不良反应发生率为13.89%,高于观察组的5.56%,差异有统计学意义(P<0.05)。结论手术结束前30 min预注纳布啡并不会影响全麻下行腹腔镜保胆取石术患者的苏醒时间和拔管时间,并且能缓解术后疼痛,预防苏醒期躁动,减少不良反应的发生率。

Objective To explore the effect of nalbuphine in the resuscitation period on patients with laparoscopic gallbladder preserving lithotomy.Methods 72 patients undergoing laparoscopic gallbladder preserving lithotomy in Tianjin Baixin Hospital from December 2015 to December 2016 were randomly divided with random number table into observation group and control group each with 36 patients.All patients were given routine anesthesia.About 30 minutes before the end of the operation,0.2 mg/kg of intravenous nalbuphine injection was given to the observation group,while the same dose of saline was given to the control group.The recovery time,extubation time,the restlessness score and adverse reaction were compared between two groups.Results The recovery time and extubation time between the two groups had not significantly different（P〉0.05）.The restlessness scores in the observation group were lower than those in the control group,the difference was statistically significant（P〈0.05）.The incidence of adverse reactions in the control group was 13.89%,up from 5.56% in the observation group,and the differences were statistically significant（P0.05）.Conclution Preventive intravenous injection of nalbuphine will not affect the recovery time and extubation time of laparoscopic gallbladder preserving lithotomy,but can effectively relieve postoperative pain,prevent emergence agitation,reduce the incidence of adverse reactions.
