三维动态标记下踝关节权重系数的定量研究

摘要 目的探讨踝关节在下肢功能中合理的权重系数。方法通过LUKOtronic三维运动捕捉系统动态标记记录健康受试者踝关节在正常状态下的下肢运动情况以及踝关节在功能位、背伸位和跖屈位强直时下肢运动情况,经Fugl-Meyer下肢运动功能评价表计算得出踝关节在上述三个不同强直位时下肢功能丧失程度,最后通过统计学方法推算踝关节合理的权重系数范围。结果踝关节在功能位、背伸位、跖屈位强直时导致的下肢功能丧失程度具有组间统计学差异;进行多重比较时,功能位强直与背伸位强直导致的下肢功能丧失程度、功能位强直与跖屈位强直导致的下肢功能丧失程度两组比对结果均具有统计学差异;在95%的置信区间,踝关节在下肢功能中的权重系数范围为45.78%~53.26%。结论定义不合理的权重系数以及独立考虑踝关节功能而忽视踝关节与其他关节的协调作用都不是评估下肢功能的科学方法。 Objective To study the reasonable weight coefficient of ankle in the lower extremity. Methods LUKOtronic 3-D dynamic identification method was used to record the healthy testers' lower extremity movements while the ankle movement was normal and when the ankle was ankylosis at functional position, dorsiflex and plantarflexion position. Count the lower extremity losses by using Fugl Meyer assessment form when the ankle was ankylosis at functional position, dorsiflex and plantarflexion position, speculating the reasonable weight coefficient of ankle by statistic methods. Results There were statistical differences among the lower extremity losses when the ankle was ankylosis at functional position, dorsiflex and plantarflexion position, When comparing any two of three means,, there was statistical differences between the lower extremity when the ankle was ankylosis at functional and dorsiflex position, also when the ankle was ankylosis at functional and plantarflexion position. With the 95% confidence interval, the reasonable weight coefficient of ankle wasfrom 45.78% to 53.26%. Conclusion Neither of defining an unreasonable weight coefficient nor ignoring the ankle coordinating function with other joints was a scientific approach to evaluate the lower extremity function.
