我院临床药师2016年至2017年会诊回顾性分析

Retrospective Analysis of Clinical Pharmacists' Participating in Clinical Consultations in Our Hospital from 2016 to 2017

目的:总结曲靖市第一人民医院近两年来临床药师参与临床会诊的实践经验,为进一步提高药学会诊质量提供参考。方法:对2016年至2017年曲靖市第一人民医院临床药师参与的所有会诊进行回顾性分析,内容包括科室分布、会诊原因、会诊模式、建议采纳、病情转归等。结果:两年曲靖市第一人民医院临床药师共参与会诊2 309例,来自23个临床科室,申请药师会诊的原因主要是请求指导抗菌药物使用,占全部会诊的98.45%,普通会诊、特殊使用级抗菌药物会诊、急会诊和全院会诊的占比分别为41.92%、55.99%、0.87%和1.22%。94.79%的会诊建议被采纳,在采纳建议中73.14%的病例治疗有效。结论:临床药师参与会诊已经取得了一定的成效,但仍需要不断学习知识,在实践中不断总结经验,提高会诊建议有效率和会诊质量,促进临床合理用药。

Objective： To summarize the clinical experience of clinical pharmacists＇ participating in clinical consultations in the First People＇s Hospital of Qujing in recent 2 years in order to offer references for further improvement of the quality of medical consultations.Methods：Made retrospective analysis of the clinical pharmacists＇ participating in the clinical consultation in the First People＇s Hospital of Qujing from 2016 to 2017, including the department distribution,reasons of consultation,consultation modes,the adoption of suggestions and results of outcomes. Results：Within two years the total number of consultation cases in which the clinical pharmacists from 23 clinical departments participated was up to 2 309. The major reason for clinical consultations was asking for instructions in the use of antibacterial agents, amounting for 98.45%. The proportions of the common consultation, the special one, the emergency one and the multidisciplinary one amounted for 41.92%, 55.99%, 0.87% and 1.22% respectively. 94.79% of the consultation suggestions was adopted and 73.14% cases had received effective treatments after taking the suggestions. Conclusion：Clinical pharmacists＇ participating in medical consultations has obtained some certain progress, but further study is still needed and more experience needs to be summarized in practice so that the efficiency and quality of consultations can be further improved and the clinical medicines can be used rationally in the future.
