江苏兴化地区妊娠早期妇女25-羟维生素D水平分析

摘要 目的:了解江苏兴化地区妊娠早期妇女25-羟维生素D[25-(OH)D]的水平.方法:选取2017年1月~2017年12月,在兴化市人民医院产科门诊,进行首次产检的妊娠早期(0~12+6周)妇女886例,年龄17~44岁,平均(27.34±4.50)岁.采用高效液相色谱串联质谱法检测25-(OH)D水平.结果:886例妊娠早期妇女25-(OH)D的平均水平(20.33±8.26)ng/ml.其中缺乏状态(〈20ng/ml)480例,占54.2%;不足状态(20.1~30ng/ml)283例,占31.9%;正常状态(30.1~100ng/ml)123例,占13.9%.按照年龄分组中,≤20岁水平最低(18.50±8.87)ng/ml,31~35岁水平最高(22.52±8.25)ng/ml.各年龄组间差异无统计学意义(P〉0.05).按照月份分组,2月份水平最低(15.73±6.34)ng/ml,8月份水平最高(25.09±8.60)ng/ml,差异无统计学意义(P〉0.05).各月25-(OH)D水平与平均气温具有相关性(r=0.846,P〈0.05);与日照时数不具有相关性(r=0.231,P〉0.05).结论:江苏兴化地区妊娠早期妇女25-(OH)D总体水平偏低,存在缺乏和不足的占86.1%;年龄分组中≤20岁的水平最低;各月分组中2月份水平最低.临床应引起高度重视,采取预防措施,避免维生素D缺乏引起的母婴并发症;同时加强健康宣教,提高育龄妇女对维生素D不足现状的认识. Objective: To understand the level of 25- hydroxyvitamin D[25-(OH)D] in the early pregnant women of Xinghua, Jiangsu. Methods: From January 2017 to December 2017, in the obstetric outpatient department of Xinghua People's Hospital, 886 women aged 17-44 years old with an average age of (27.34 ± 4.50) years in the first trimester of pregnancy (0-12+6 weeks) were selected. The level of 25-(OH)D was detected by high performance liquid chromatography tandem mass spectrometry. Results: The average level of 25-(OH) D was (20.33± 8.26) ng/ml in 886 early pregnant women. Among them, there were 480 cases with lacking state (〈20ng/ml), accounting for 54.2%, 283 cases (20.1-30ng/ml) with deficiency, accounting for 31.9%, and 123 cases in normal condition (30.1-100ng/ml), accounting for 13.9%. According to the age group, the group of less than 20 years has the lowest level (18.50 ±8.87) ng/ml, 31-35 years group has the highest level (22.52 ± 8.25) ng/ml. There was no significant difference between different age groups (P〉0.05) . According to the month group, the lowest level was in February, (15.73± 6.34) ng/ml, and the highest level was in August, (25.09 ± 8.60) ng/ml, and the difference was not statistically significant (P〉0.05). The level of 25-(OH)D in each month was correlated with the average temperature (r=0.846, P 〈 0.05) , and did not have a correlation with the sunshine hours (r=0.231, P〉0.05) . Conclusion: The overall level of 25-(OH)D in early pregnant women of Jiangsu Xinghua area is low, lack or deficiency rate accounted for 86.1%; the age group with the lowest level was less than 20 years group. The month group with the lowest level was February. We should attach great importance to clinical practice and take preventive measures to avoid maternal and infant complications caused by vitamin D deficiency. At the same time, we should strengthen health education and raise awareness of vitamin D deficiency among women of childbearing age.
