摘要
目的:观察应用关节松动训练联合体外冲击波靶点治疗肩关节周围炎的疗效及其安全性。方法:选取肩关节周围炎治疗的患者44例,随机分为对照组21例和观察组23例,对照组采用肩关节松动训练治疗,观察组在前者基础上联合体外冲击波靶点治疗。采用视觉模拟评分(VAS)对患者治疗前、治疗7、14、21和28d后的疼痛程度进行评估。采用Constant肩关节功能评分(CMS)和肩关节活动度(ROM)对患者治疗前和治疗结束后的功能情况进行评估。结果:2组患者治疗前与治疗后不同时间点的VAS评分比较,差异有统计学意义(均P<0.05);2组间治疗后7d及14d后比较,观察组的VAS评分均低于对照组,但差异无统计学意义;治疗后21和28d后,观察组VAS评分明显低于对照组(均P<0.05)。治疗28d后,观察组与对照组CMS评分及ROM均高于治疗前(均P<0.05),且观察组更高于对照组(均P<0.05)。结论:关节松动训练联合体外冲击波靶点治疗肩关节周围炎临床疗效可,值得临床治疗过程中推广应用。
Objective:To explore the effectiveness and safety of joint loose training/mobilization combined with extracorporeal shock wave on scapulohumeral periarthritis.Methods:Forty-four patients with scapulohumeral periarthritis were randomly divided into control group and treatment group.Twenty-one patients in the control group received joint loose training,and 23 patients in the treatment group received joint loose training/mobilization combined with extracorporeal shock wave.The visual analogue scale(VAS)was used before and 7 th,14 th,21 st and 28 th day after treatment.The Constant-Murley shoulder assessment score(CMS)and the range of motion(ROM)were applied to assess the function of shoulder before and after treatment.Results:The VAS,CMS,and ROM scores were significantly improved respectively after treatment in both groups(P0.05).The VAS scores at 7 th,14 th,21 st and 28 th day after treatment showed more reductions in the treatment group than in the control group.At 21 st and28 th day after treatment,the VAS scores in the treatment group were significantly different from those in the control group(P0.05).Conclusion:The joint loose training/mobilization combined with extracorporeal shock wave may have significant efficacy and safety in treating scapulohumeral periarthritis,and be worth expanding in clinical applications.
