营养支持对老年慢性阻塞性肺疾病急性加重期患者临床疗效

Clinical effect of nutritional support on elderly patients with acute exacerbation of chronic obstructive pulmonary disease

目的研究营养支持对老年慢性阻塞性肺疾病急性加重期患者疗效。方法选取广饶经济开发区卫生院2017年2月至12月收治的82例老年慢性阻塞性肺疾病急性加重期患者,按照随机数字表法分为对照组和观察组(各41例)。给予患者常规抗感染,平喘祛痰治疗,在此基础上对照组采用常规饮食治疗,观察组采用营养支持治疗,观察对比两组营养情况、心肺功能及治疗效果。结果治疗前两组患者营养情况各指标相比无差异,P>0.05;经治疗后两组患者血清总蛋白、白蛋白、转铁蛋白以及红蛋白数值较治疗前均明显上升,且观察组各项指标均明显优于对照组,P<0.05。治疗前两组患者心肺功能各指标相比无差异,P>0.05;治疗后两组患者PaCO_2数值均明显下降,LVEF,PaO_2,PAMP数值均明显上升,且观察组明显优于对照组,P<0.05。治疗后观察组脱机时间、住院天数、CAT评分均明显低于对照组,P<0.05。结论营养支持可增强老年慢性阻塞性肺疾病急性加重期患者营养,增强患者心肺功能,促进患者恢复。

Objective To explore the effect of nutritional support on elderly patients with acute exacerbation of chronic obstructive pulmonary disease. Methods 82 cases of elderly patients with acute exacerbation of chronic obstructive pulmonary disease were selected from 2017. 2 ~ 2017. 12 Hospital of Guangrao economic development zone,they were randomly divided into control group and observation group（41 cases in each group）. The patients were treated with routine anti infection and antiasthmatic and expectorant treatment. On this basis,the control group was treated with conventional diet,and the observation group was treated with nutritional support,and the nutritional status,cardiopulmonary function and treatment effect of the two groups were observed and compared. Results There was no difference in nutritional status between the two groups before treatment（P〉0. 05）; After treatment,the total serum protein,albumin,transferrin and erythroprotein in the two groups were significantly higher than those before treatment,and the indexes of the observation group were obviously superior to those of the control group,P〈0. 05; Before and after treatment,there was no difference in cardiopulmonary function between the two groups,P〉0. 05; After treatment,the PaCO_2 values of the two groups decreased significantly,while the values of LVEF,PaO_2 and PAMP increased significantly,and the observation group was significantly better than the control group,P〈0. 05; After treatment,the weaning time,days of hospitalization and CAT scores in observation group were significantly lower than those in control group,P〈0. 05. Conclusion Nutritional support can enhance the nutrition of elderly patients with acute exacerbation of chronic obstructive pulmonary disease,enhance their cardiorespiratory function,and promote their recovery.
