米非司酮配伍米索前列醇及雌激素治疗稽留流产临床分析

Clinical Analysis of Mifepristone Combined with Misoprostol and Estrogen for Treatment of Missed Abortion

目的探讨分析米非司酮配伍米索前列醇及雌激素治疗稽留流产的临床效果。方法采用随机分组的方式将2016年5月—2018年2月间诊治的70例稽留流产患者分为两组,对照组采取戊酸雌二醇口服+人工流产钳刮术,观察组给予米非司酮+米索前列醇+戊酸雌二醇治疗,观察两组的流产效果及药物不良反应。结果观察组完全流产率为68.6%,不全流产率为22.9%,流产效果显著优于对照组(χ~2=3.733、5.040,P=0.045、0.022);两组流产失败率、用药不良反应发生率差异无统计学意义(χ~2=0.215、0.729,P=0.500、0.337)。观察组不全流产者行清宫术中宫颈扩张率、二次清宫术及术后人流综合征发生率更低,手术用时更短,出血量更少,术后出血时间更短(P<0.05)。结论米非司酮配伍米索前列醇及雌激素治疗稽留流产效果满意,安全性高,对患者机体损伤小,有利于术后恢复。

Objective To investigate the clinical effects of mifepristone combined with misoprostol and estrogen in the treatment of missed abortion. Methods A total of 70 patients with missed abortion diagnosed between May 2016 and February 2018 were randomly divided into two groups. The control group received estradiol valerate oral ＋ abortion curettage and the observation group received the treatment of non-steroids ＋ misoprostol ＋ estradiol valerate, to observe the effects of abortion and adverse drug reactions in the two groups. Results The complete abortion rate of the observation group was 68.6%, and the incomplete abortion rate was 22.9%. The abortion effect was significantly better than that of the control group（χ^2=3.733, 5.040, P=0.045, 0.022）; the incidence of miscarriage failure rate and adverse drug reactions in the two groups, the difference was not statistically significant（χ^2=0.215, 0.729, P=0.500, 0.337）. The rate of cervical dilatation, secondary curettage, and postoperative abortion syndrome in patients with incomplete abortion in the observation group were lower, the duration of surgery was shorter, the amount of bleeding was less, and postoperative bleeding time was shorter（P〈0.05）. Conclusion Mifepristone combined with misoprostol and estrogen have satisfactory effects on the treatment of missed abortion, high safety, little damage to the body of the patient, and are beneficial for postoperative recovery.
