时间-空间相关成像技术在评估胎儿心脏功能的初步研究 被引量:3

摘要 目的研究时间-空间相关成像技术(spatio-temporal image correlation,STIC)在评估胎儿心脏功能中的价值。方法选取川北医学院附属医院2015年1月—2017年12月常规二维超声显示胎儿心脏结构无异常的200例单胎妊娠孕妇为研究对象,均行STIC检查,测量、计算不同孕周胎儿左右心室收缩末期容积(end systolic volume,ESV)、舒张末期容积(end diastolic volume,EDV)、射血分数(ejection f raction,EF)、每搏量(st roke volume,SV)、心排血量(cardiac out put,CO),对心脏相关指标与孕周的关系行Pearson相关分析。结果左心室ESV从21~22+6周的(0.41±0.09)ml增多到31~32+6周的(1.10±0.14)ml,右心室ESV从(0.50±0.11)ml增多到(1.20±0.12)ml,左右心室EDV分别从21~22+6周的(0.76±0.15)ml、(1.03±0.12)ml增多到31~32+6周的(2.31±0.20)ml、(2.48±0.20)ml,左右SV、CO也随孕周增长而增多;Pearson相关分析显示ESV、EDV、SV、CO均与孕周正相关(P<0.05)。结论 STIC通过胎儿心室容积测量可精确显示胎儿心功能。 ObjectiveTo study the value of spatiotemporal image correlation(STIC)in evaluating fetal cardiac function. MethodsTwo hundred women with single pregnancy and without abnormal fetal cardiac structure examined by conventional two-dimensional ultrasound in the Affiliated Hospital of North Sichuan Medical College between January 2015 and December 2017 were selected as the subjects. They were examined by STIC. The left and right ventricular end systolic volume(ESV), end diastolic volume(EDV), ejection fraction(EF), stroke volume(SV)and cardiac output(CO)of the fetuses were measured and calculated in different gestational weeks. The relationships between cardiac related indexes and gestational weeks were analyzed with Pearson analysis. ResultsThe left ventricular ESV increased from(0.41±0.09)ml at 21~22+6 weeks to(1.10±0.14)ml at 31~32+6 weeks, while the right ventricular ESV increased from(0.50±0.11)ml to(1.20±0.12)ml. The left and right ventricular EDV increased from(0.76±0.15)ml and(1.03±0.12)ml at 21~22+6 weeks to(2.31±0.20)ml and(2.48±0.20)ml at 31~32+6 weeks. The left and right SV and CO also increased with the gestational week. Pearson correlation analysis showed that ESV, EDV, SV and CO were associated with the gestational week(P〈0.05). ConclusionSTIC can accurately display the fetal cardiac function by measuring the fetal ventricular volume.
