枸橼酸咖啡因对呼吸暂停早产儿的血气和β-内啡肽的改善作用 被引量:6

摘要 目的探究枸橼酸咖啡因在早产儿呼吸暂停的临床作用。方法选择2014年3月—2017年3月在葛洲坝集团中心医院接受诊治的126例早产呼吸暂停患儿,随机分为对照组和枸橼酸咖啡因组(简称枸橼酸组),每组63例,对照组患儿行氨茶碱和纳洛酮治疗,枸橼酸组行枸橼酸咖啡因治疗。结果枸橼酸组有效率为73.02%,对照组为66.67%,枸橼酸组治疗有效率高于对照组(P<0.05)。枸橼酸组药物起效时间早于对照组(P<0.05),且其呼吸暂停次数和时间均短于对照组(P<0.05)。治疗前后,2组患儿血氧饱和度(SaO_2)、血氧分压(PaO_2)、血二氧化碳分压(PaCO_2)比较差异均无统计学意义(P>0.05)。治疗后,枸橼酸组β-内啡肽表达水平高于对照组(P<0.05)。枸橼酸组总体药物不良反应情况低于对照组(P<0.05)。结论针对早产儿呼吸暂停的治疗,枸橼酸咖啡因具有较好的有效性和安全性,具有较高的推广和应用价值。 Objective To explore the clinical effect of caffeine citrate against apnea in preterm infants. MethodsOne hundred and twenty-six cases of premature apnea in children treated between March 2014 and March 2017 were randomly divided into the control group and caffeine citrate group(citrate group), with 63 cases in each. The control group was treated with aminophylline and naloxone, while the citrate group was given caffeine citrate. ResultsThe effective rate of the citrate group was 73.02%, compared with 66.67% in the control group, so the effective rate of the citrate group was higher than that of the control group(P〈0.05). The onset time of the citrate group was significantly earlier than that of the control group(P〈0.05). The frequency and duration of apnea were lower or shorter than those of the control group(P〈0.05). Before and after treatment, there was no significant difference in the indexes of PaO2, PaCO2 or SaO2 between the two groups(P〉0.05). After treatment, the expression of β-endorphin in the citrate group was higher than that of the control group(P〈0.05). The incidence of adverse drug reactions of the citrate group was lower than that of the control group(P〈0.05). ConclusionIn the treatment of premature infants with apnea, caffeine citrate not only has good efficacy and safety, but also is worthy of popularization and application.
