无创机械通气在老年肺动脉高压患者呼吸衰竭中的应用

Application of Noninvasive Mechanical Ventilation in Treatment of Respiratory Failure in Elderly Patients With Pulmonary Hypertension

目的探讨无创机械通气在老年肺动脉高压患者呼吸衰竭治疗中的应用效果。方法选取我院2015年1月—2017年1月收治的86例老年肺动脉高压并发呼吸衰竭患者为研究对象,将本组患者随机分为两组,观察组43例给予无创机械通气,对照组43例给予有创机械通气,对比两组患者治疗前后氧合功能。结果治疗后观察组患者OI、Pa O_2明显高于对照组,Sa O_2、Pa CO_2明显低于对照组,差异具有统计学意义(P<0.05)。两组患者不良反应发生率对比差异具有统计学意义(P<0.05)。结论无创机械通气可有效改善老年肺动脉高压患者呼吸衰竭氧合功能,临床效果满意。

Objective To explore the effect of noninvasive mechanical ventilation in the treatment of respiratory failure in elderly patients with pulmonary hypertension.Methods 86 cases of senile pulmonary arterial hypertension complicated with respiratory failure were selected in our hospital from January 2015 to January 2017.The patients were randomly divided into two groups,43 cases in the observation group were given non-invasive mechanical ventilation,43 cases in the control group were given invasive mechanical ventilation,and the oxygenation function of the two groups before and after treatment was compared.Results After treatment,the OI and Pa O_2 in the observation group were significantly higher than those in the control group,while Sa O_2 and Pa CO_2 were significantly lower than those in the control group,and the difference was statistically significant（P 〈0.05）.The difference of incidence of adverse reactions between the two groups was statistically significant（P 〈0.05）.Conclusion Noninvasive mechanical ventilation can effectively improve the oxygenation function of respiratory failure in elderly patients with pulmonary hypertension,and the clinical effect is satisfactory.
