Damon Q自锁托槽联合Invisalign矫治器治疗替牙中晚期牙性错颌畸形患者的临床效果

Clinical effects of Damon Q self locking brackets combined with Invisalign appliance on middle and advanced dentofacial malocclusion

目的探讨Damon Q自锁托槽联合Invisalign矫治器治疗210例替牙中晚期牙性错颌畸形患者的临床效果。方法替牙中晚期的牙性错颌畸形患者210例,随机分为对照组和试验组,对照组单纯采用Damon Q自锁托槽治疗,试验组采用Damon Q自锁托槽+Invisalign矫治器进行治疗,分析牙齿移动速率、牙周维护、临床操作、患者舒适度、美观度、复诊周期、疗程、X线投影测量数据。结果 2组牙龈指数、菌斑指数及探诊深度测量比较差异无统计学意义(P>0.05),治疗后2组患者菌斑指数和牙龈指数均有显著提高,试验组患者数据增加更为显著,差异均有统计学意义(P<0.05);治疗前2组患者SN-MP、Y轴角、L1-MP差异不明显,治疗后试验组患者夹角改善度效果优于对照组(P<0.05);试验组患者平均治疗时间明显短于对照组,复诊间隔时间延长优于对照组,差异均有统计学意义(P<0.05);在满意度调查方面,试验组满意率显著优于对照组,差异有统计学意义(P<0.05)。结论 Damon Q自锁托槽联合Invisalign矫治器治疗替牙中晚期牙性错颌畸形患者,降低患者牙龈炎的发病率,较少患者复诊次数并使患者获得舒适的矫治体验,提高正畸医生工作效率,取得了良好的临床疗效,得到了医生和患者的一致好评,值得临床推广。

Objective To investigate the therapeutic effects of Damon Q self locking brackets combined with Invisalign appliance on middle and advanced dentofacial malocclusion. Methods A total of 210 patients with middle and advanced dentofacial malocclusion who were treated in our hospital from March 2012 to December 2016 were randomly divided into control group and experimental group. The patients in control group were treated by Damon Q self locking brackets,however,the patients in experimental group were treated by Damon Q self locking brackets combined with Invisalign appliance. The tooth movement velocity,periodontal maintenance,clinical operation,patient ＇s comfort degree,beautiful outlook degree,return visit cycle,treatment course and cephalometric data were observed and compared between the two groups. Results There were no significant differemnces in gingival index,plaque index and probing depth measurement data between the two groups before treatment（P〈0. 05）. After treatment the plaque index and gingival index were significantly improved in both groups,moreover,which in experimental group were superior to those in control group（P〈0. 05）. Before treatment there were no significant differemnces in SN-MP,Y,L1-MP axis angle between the two groups（P〈0. 05）. After treatment the improvement degree of included angle in experimental group was superior to that in control group（P〈0. 05）.Moreover the average treatment time in experimental group was（456. 48 ± 56. 19） days,which was significantly shorter than that[（591. 93 ±82. 64） days] in control groups,and the return visit interval time in experimental group was superior to that in control group（P〈0. 05）. In addition the satisfaction degree of patients in experimental group was superior to that in control group（P〈0. 05）. Conclusion Damon Q self locking brackets combined with Invisalign appliance in treatment of middle and advanced dentofacial malocclusion can reduce the incidence rate of gingivitis,lessen return visit frequency of patients and improve satisfaction degree od patients,and can obtain significant therapeutic effects.
