比索洛尔治疗老年慢性心功能不全合并心肌梗死的疗效及安全性

Efficacy and safety of bisoprolol in the treatment of elderly patients with chronic cardiac insufficiency complicated with myocardial infarction

目的探讨比索洛尔治疗老年慢性心功能不全合并心肌梗死患者的临床疗效及安全性。方法选取82例老年慢性心功能不全合并心肌梗死患者,依据随机数字表法分为对照组(n=41)和观察组(n=41)。对照组患者采用临床常规治疗,观察组患者在常规治疗的基础上加用比索洛尔治疗。比较2组患者治疗前后血压、心率、左室收缩末期容积(LVESV)、左室舒张末期容积(LVEDV)、左室射血分数(LVEF)和B型尿钠肽(BNP)水平变化。结果治疗后,2组患者血压水平无显著改善,但心率均显著下降,且观察组患者心率下降幅度显著优于对照组(P<0.05);治疗后,2组患者LVESV、LVEDV、LVEF及BNP水平比较,差异有统计学意义(P<0.05)。结论比索洛尔治疗老年慢性心功能不全合并心肌梗死患者的疗效显著,可改善患者的心率,提高心功能,值得临床推广。

Objective To analyze clinical efficacy and safety of bisoprolol in the treatment of elderly patients with chronic heart failure complicated with myocardial infarction. Methods Eighty-two elderly patients with chronic cardiac insufficiency complicated with myocardial infarction treated in our hospital were divided into control group（ n = 41） and observation group（ n = 41） according to random number table method. Patients in the control group were treated with routine clinical treatment,while those in the observation group were treated with bisoprolol on the basis of routine treatment. The changes of blood pressure,heart rate,left ventricular end-systolic volume（ LVESV）,left ventricular end-diastolic volume（ LVEDV）,left ventricular ejection fraction（ LVEF）,and B-type natriuretic peptide（ BNP） levels before and after treatment were compared between the two groups.Results After the treatment,the blood pressure levels of the two groups had no significant improvement,but the heart rate decreased significantly,and the observation group decreased more significantly（ P〈0. 05）. After treatment,the LVESV,LVEDV,LVEF,and BNP levels showed significant differences between two groups（ P〈0. 05）. Conclusion Bisoprolol in elderly patients with chronic cardiac insufficiency combined with myocardial infarction can effectively improve heart rate and heart function,has significant efficacy.
