急性冠状动脉综合征患者髓样相关蛋白8/14水平及其临床意义

Myeloid-related protein 8/14 level in acute coronary syndrome and its clinical significance

目的探讨急性冠状动脉综合征(ACS)患者髓样相关蛋白8/14(MRP 8/14)水平及其临床意义。方法选择100例ACS患者为研究组,100例健康体检者为对照组。比较两组血清MRP 8/14的表达水平。采用受试者工作特征(ROC)曲线判断MRP 8/14对ACS的诊断价值。根据最佳临界值将ACS患者分为MRP 8/14低值组和MRP 8/14高值组,比较两组的临床资料和死亡情况。采用Cox模型分析MRP 8/14对ACS患者随访期间死亡的影响。结果研究组MRP 8/14表达水平高于对照组(P<0.05)。ROC曲线分析结果显示,MRP 8/14诊断ACS的曲线下面积为0.793(P<0.05),最佳临界值为1 526.2 ng/ml。MRP 8/14高值组患者年龄、糖尿病比例、Killip分级>1比例、肌酸激酶同工酶MB峰值和心肌肌钙蛋白T峰值均高于低值组(P<0.05)。随访1年,MRP 8/14低值组死亡比例低于MRP 8/14高值组(P<0.05)。Cox分析结果显示,MRP 8/14水平升高为ACS患者随访期间死亡的危险因素(P<0.05)。结论 MRP 8/14在ACS患者中表达上调,其对诊断ACS及评估预后有重要意义。

Objective To investigate the level of myeloid-related protein 8/14（MRP 8/14） in acute coronary syndrome（ACS） and its clinical significance. Methods One hundred patients with ACS were enrolled as study group,and another 100 healthy people as control group.The serum expression levels of MRP 8/14 were compared between the two groups.The diagnostic value of MRP 8/14 for ACS was determined by receiver operating characteristic（ROC） curve.The ACS patients were divided into low-value MRP 8/14 group and high-value MRP 8/14 group depending on the optimum cutoff value of MRP 8/14.And the clinical data and mortality were compared between the two groups.Cox model was used to analyze the effect of MRP 8/14 on the mortality of ACS patients during the follow-up. Results The expression level of MRP 8/14 in the study group was higher than that in the control group（ P 〈0.05）.The results of ROC curve analysis showed that the area under the curve of MRP 8/14 for diagnosing ACS was 0.793（ P 〈0.05）,with a optimum cutoff value of 1 526.2 ng/ml.The patients in the high-value MRP 8/14 group were older,and had higher proportions of diabetes and Killip grade 〉1 ,and higher peak creatinine kinase-MB isoenzyme and peak cardiac troponin T compared to the low-value MRP 8/14 group（ P 〈0.05）.During the 1-year follow-up,the mortality of the low-value MRP 8/14 group was lower than that of the high-value MRP 8/14 group（ P 〈0.05）.The results of Cox analysis showed that elevated level of MRP 8/14 was the risk factor for the death of ACS patients during follow-up. Conclusion MRP 8/14 expression is up-regulated in ACS patients,and it′s valuable for the diagnosis and prognosis assessment of ACS.
