M2丙酮酸激酶对炎症性肠病诊断价值的Meta分析

摘要 目的系统评价M2丙酮酸激酶(M2-PK)在炎症性肠病(IBD)诊断中的临床价值。方法计算机检索Pub Med、EMbase、Web of Science、Cochrane图书馆、中国期刊全文数据库、万方数字化期刊全文数据库中有关M2PK对IBD诊断价值的研究。筛选并对纳入文献进行质量评价,采用Meta-Disc 1.40软件对数据进行Meta分析。结果共纳入10篇文献,其中IBD组518例,对照组544例。Meta分析分析结果显示,M2-PK诊断IBD的汇总灵敏度为0.79,特异度为0.73,综合受试者工作特征(SROC)曲线下面积为0.8581,Q*为0.79889;M2-PK诊断溃疡性结肠炎的SROC曲线下面积为0.8755,Q*为0.8060;M2-PK诊断克罗恩病亚的灵敏度为0.83,特异度为0.67,SROC曲线下面积为0.8637,Q*为0.7943。结论 M2-PK对诊断IBD有较高灵敏度和特异度,可用于临床辅助诊断IBD。 Objective To systemically evaluate the clinical value of M2 pyruvate kinase( M2-PK) in the diagnosis of inflammatory bowel disease( IBD). Methods A computer retrieval was performed in the databases of Pub Med,EMbase,Web of Science,Cochrane Library,CNKI database and Wan Fang database for the study on the diagnostic value of M2-PK for IBD. A quality assessment was conducted in the selected literatures. Meta-Disc 1. 40 software was applied to a Meta-analysis. Results A total of 10 studies were included,involving518 IBD patients and 504 controls. The results of the Meta-analysis showed that the combined sensitivity of M2-PK for diagnosing IBD was0. 79,the specificity was 0. 73,the area under the summary receiver operating characteristic( SROC) curve was 0. 8581,and the Q*was0. 79889. The area under the SROC curve of M2-PK for diagnosing ulcerative colitis was 0. 8755,and the Q*was 0. 8060. The sensitivity,specificity,area under the SROC curve and the Q*of M2-PK for diagnosing Crohn's disease were 0. 83,0. 67,0. 8637 and 0. 7943 respectively.Conclusion M2-PK has high sensitivity and specificity in the diagnosis of IBD,and can be used in the adjuvant clinical diagnosis of IBD.
