摘要
目的调查结肠癌患者肠道菌群分布情况亲分析其临床意义。方法选取85例结肠癌患者为结肠癌组,另选取35例结肠腺瘤患者和35例健康人群作为对照,采用培养法检查粪便菌群分布,并进行比较分析。手术前后采集结肠癌组患者静脉血,采用ELISA法检测血清D-乳酸、内毒素水平,并与两对照组检测结果相比较。结果结肠癌组患者术前的大肠埃希菌、粪肠球菌、弯曲杆菌、双歧杆菌、乳酸杆菌活菌集落数[(7.71±1.59)Log10、(6.93±1.69)Log10、(7.37±1.49)Log10、(7.43±0.89)Log10、(7.37±1.31)Log10]和术后的大肠埃希菌、粪肠球菌、弯曲杆菌、双歧杆菌、乳酸杆菌活菌集落数[(8.68±1.68)Log10、(7.85±1.76)Log10、(5.97±1.53)Log10、(6.12±1.14)Log10、(5.83±1.25)Log10]与结肠腺瘤组和健康对照组比较差异有统计学意义(P<0.05);结肠癌患者术后与术前比较大肠埃希菌、粪肠球菌、弯曲杆菌、双歧杆菌、乳酸杆菌活菌集落数差异有统计学意义(P<0.05)。结肠腺瘤组与健康对照组比较大肠埃希菌、粪肠球菌、弯曲杆菌、双歧杆菌、乳酸杆菌活菌集落数[(6.85±0.97)Log10vs(6.01±0.62)Log10、(6.61±0.92)Log10vs(5.59±0.68)Log10、(7.95±1.52)Log10vs(8.71±1.28)Log10、(8.41±1.08)Log10 vs(9.24±1.07)Log10、(7.96±1.42)Log10vs(8.72±1.22)Log10]差异有统计学意义(P<0.05)。结肠癌组患者手术前后血清D-乳酸、内毒素水平均显著高于健康对照组(P<0.05);结肠癌组患者手术前后血清D-乳酸和内毒素水平差异均有统计学意义(P<0.05)。结论结肠癌患者存在肠道菌群失调,且术后更为严重。调节肠道菌群可能有利于结肠癌的防治。
Objective To investigate the distribution and clinical significance of intestinal flora in patients with colon cancer. Methods Eighty-five patients with colon cancer served as the colon cancer group,and 35 patients with adenoma of the colon and 35 healthy people served as control groups.The distribution of faecal flora was detected by culturing,and the results were analyzed and compared.The venous blood of patients with colon cancer was collected before and after surgery.The levels of D-lactic acid and endotoxin were detected with ELISA,and the results were compared to the two control groups. Results Before surgery,the Escherichia coli colony count was 7.71±1.59 Log10,the Enterococcus faecalis colony count was 6.93±1.69 Log10,the Campylobacter coli colony count was 7.37±1.49 Log10,the Bifidobacterium coli colony count was 7.43±0.89 Log10,and the Lactobacillus coli colony count was 7.37±1.31 Log10.After surgery,the E.coli colony count was 8.68±1.68,the E.faecalis colony count was 7.85±1.76 Log10,the Campylobacter coli colony count was 5.97±1.53 Log10,the Bifidobacterium coli colony count was 6.12±1.14 Log10,and the Lactobacillus coli colony count was 5.83±1.25 Log10.The colony counts in patients with adenoma of the colon and the healthy control group differed significantly(P〈0.05).Detection of E.coli,E.faecalis,Campylobacter,Bifidobacterium,and Lactobacillus in the colon cancer group differed significantly before and after surgery(P〈0.05).Detection of E.coli,E.faecalis,Campylobacter,Bifidobacterium,and Lactobacillus differed significantly in patients with adenoma of the colon and the healthy control group(P〈0.05).The levels of D-lactic acid and endotoxin in the colon cancer group before and after surgery were significantly higher than those in the healthy control group(P〈0.05).Levels of D-lactic acid and endotoxin in the colon cancer group differed significantly before and after surgery(P〈0.05). Conclusion Intestinal flora is disrupted in patients with colon cancer,and this worsens after surgery.The regulation of intestinal flora helps to control colon cancer.
