心脉隆注射液对脓毒症患者心肌损伤及细胞凋亡水平的影响 被引量:3

摘要 目的探讨心脉隆注射液对脓毒症患者心肌损伤及细胞凋亡水平的影响。方法纳入本院ICU脓毒症患者51例,实验组在常规治疗基础上,给予心脉隆注射液100 mg+5%葡萄糖注射或0.9%氯化钠注射液稀释至50 ml,15~30 ml/h速度静脉泵入,每日2次,间隔时间6 h以上,5日为一疗程,常规治疗组为对照组。观察两组患者入ICU即刻,1 d,5 d血清心肌损伤的分子标记物肌钙蛋白I(c Tn I),肌酸激酶(CK),血清BNP及细胞凋亡因子血清s Fas水平。结果脓毒症患者入ICU时血清CK、c Tn I、BNP、s Fas水平均显著高于正常人,入ICU第一天试验组c Tn I水平显著低于对照组,两者比较具有显著性差异(P<0.05)。实验组第5天,血清CK、c Tn I、BNP均显著低于对照组,两者比较具有显著性差异(P<0.05)。脓毒症血清s Fas水平明显高于正常人,脓毒症患者第5天,血清s Fas水平较前显现下降。实验组第1天与第5天血清s Fas水平明显低于对照组,两者比较具有显著性差异(P<0.05)。结论心脉隆注射液能有效地保护脓毒症导致的心肌损伤,抑制细胞凋亡的发生。 Objective To explore the influence of xinmailong injection on myocardial injury and cell apoptosis of patients with sepsis.Methods A total of 51 patients who were diagnosed sepsis in our hospital,were randomized into two groups:control group(provided with routine treatment),Xinmailong group(provided with routine treatment plus Xinmailong Injection 100 mg + 5% glucose injection or 0.9% sodium chloride injection diluted to 50 ml,15-30 ml/h velocity vein pump,2 times per day,with a time interval of more than 6 h,for 5 days).The changes of molecular markers of myocardial injury including troponin I(c Tn I),creatine kinase(CK),serum BNP and serum s Fas level(apoptosis factor) were observed in the two groups at the time points admitted to the ICU immediately,1 st day and 5 th day after admittance.Results When admitted in hospital immediately,serum levels of CK,c Tn I,BNP and s Fas were significantly higher in patients with sepsis than those in health people.On the first day in ICU,the c Tn I in Xinmailong group was significantly lower than that in the control group(P〈0.05).The serum levels of CK,c Tn I and BNP in Xinmailong group were significantly lower than those in the control group at 5 th day.The serum s Fas level of sepsis patients was significantly higher than that of normal patients,and the serum s Fas level of patients with sepsis decreased significantly at the 5 th day.The serum s Fas level of Xinmailong group was significantly lower than in the control group at 1 st day and 5 th day(P〈0.05).Conclusions The xinmailong injection can effectively protect the myocardial injury caused by sepsis and inhibit the occurrence of apoptosis.
