基于云随访系统的居家管理在癌痛患者中的应用

Home management based on cloud follow-up system in patients with cancer pain

目的探讨使用云随访APP在改善出院癌痛患者服药依从性、疼痛以及满意度方面的效果。方法采用方便抽样法选择安徽医科大学第一附属医院2017年1—12月出院的癌痛患者90例，随机分为对照组（n=45）和观察组（n=45）。对照组出院后1、2、4周由责任护士给予常规电话随访，内容包括疼痛控制、用药情况、药物不良反应预防及处理等。观察组应用云随访APP软件进行疼痛管理。干预4周后比较两组的疼痛情况、服药依从性及患者满意度情况。结果干预后观察组患者服药依从性35例，对疼痛管理非常满意30例，对照组分别为23例和14例，两组比较差异有统计学意义（Z值分别为-2.928、-3.209；P〈0.01）。出院4周后，观察组疼痛数字评分（1.6±1.2）分，对照组（3.1±1.1）分，差异有统计学意义（t=6.181，P〈0.01）。结论云随访系统能提高癌痛患者的服药依从性，提高患者对护理工作的满意度，降低患者疼痛程度，值得在临床工作推广。

ObjectiveTo explore the effect of cloud follow-up system on the medication compliance, pain and degree of satisfaction in patients with cancer pain.MethodsA total of 90 patients with cancer pain who discharged from the First Affiliated Hospital of Anhui Medical University from January 2017 to December 2017 were selected by convenience sampling and were randomly divided into control group （n=45） and observation group （n=45） . In the control group, regular telephone follow-up was conducted 1 week, 2 weeks and 4 weeks after discharge by responsible nurses, including pain control, medication application, prevention and treatment of adverse drug reactions and etc. while the observation group applied cloud follow-up APP to manage pain. After intervention for 4 weeks, the pain intensity, medication compliance and patient satisfaction were compared between the two groups.ResultsIn the observation group, there were 35 cases with high compliance, 30 cases with high satisfaction for pain management comparing with 23 cases and 14 cases in the control group respectively, and the differences were statistically significant （Z=-2.928, -3.209; P〈0.01） . After intervention for 4 weeks, the points of Numeric Rating Scale in the observation group was （1.6±1.2） , and in the control group was （3.1±1.1） , and the difference was significant （t=6.181, P〈0.01） .ConclusionsThe cloud follow-up system can improve the medication compliance, patients＇ satisfaction with nursing work, and reduce the impact of pain of cancer patients. It is worth promoting in clinical work.
