癫痫儿童脑电图痫样放电与其身高的相关性分析

摘要 目的:分析癫痫儿童的身高指标及与痫样放电的关系,研究癫痫儿童的生长状况。方法:以2013~2015年在小儿神经专科门诊就诊的127例癫痫患儿作为研究对象,进行脑电图及神经影像学检查,并测量净身高。以同期在我院进行常规体检的健康儿童144例作为对照组,分别测量净身高值。参照中国《2~18岁儿童青少年身高曲线图》计算身高分布情况,进行统计学分析,探讨癫痫患儿的身高水平、矮小的发生率及其与痫样放电部位和放电指数的相关性。结果:①研究组身高分布明显集中于同年龄同性别中国健康儿童身高曲线低百分位水平,经统计学计算:癫痫患儿身高百分位分布情况与对照组比较,差异有显著意义(P〈0.001);身高低于第三百分位以下的矮小发生率为39.4%,与对照组矮小发生率11.8%比较,差异有显著意义(P〈0.05);②对不同痫样放电部位的癫痫患儿的身高进行比较,差异无显著意义(P〉0.5);且不同痫样放电部位的患儿中矮小的发生率差异也无显著意义(P〉0.5);③分析痫样放电指数〈10%、10%~30%、31%~50%、〉50%各组的身高分布及矮小的发生率,发现放电指数〉50%组,矮小的发生率明显偏高,差异有显著意义(P〈0.05)。结论:癫痫儿童身高水平普遍低于正常同龄儿童,矮小的发生率高于正常对照组。不同部位痫样放电对身高增长的影响无明显差异,但放电指数〉50%者中,身高低于正常儿童水平者比例较高,矮小的发生率也显著偏高。提示痫样放电可能对患儿身高增长有影响,值得进一步探索研究。 Objective:To analyze the relationship between the height and the epileptic discharge in children with epilepsy, and to study the growth status of children with epilepsy. Methods:As the subjects 127 cases were selected from children with epilepsy in our out-patient department of pediatric neurology from 2013 to 2015, and 144 cases healthy children were collected as the control group. All subjects' net height was measured and the standard deviation was calculated based on Chinese Growth Curve of Children and Adolescents Aged 2 to 18 Years. Statistical analysis was performed. The stature condition and incidence of short stature, and their relationship with the epileptic sample discharge and discharge index were assessed. Results:1. Compared with the control group of the same age and sex, the height of the children with epilepsy had statistically significant difference(P〈0.05). The incidence rate of the chil dren's height lower than the 3rd percentile was 39.37%, which had significant difference(P〈0.05), compared with that of unimpaired children of the same age (11.80%), and the difference was significant (P〈0.05). 2. There was no significant difference in height among children with epilepsy in different epileptic samples (P〈0.5), and there was no significant difference in the incidence of short stature. (P〈0.5). 3. Analysis of the incidence of short stature with epileptic discharge index 〈10%, 10%-30%,31%~ 50% and 〉 50 % showed that the difference was significant in the children with discharge index 50% (P〈0.05). Conclusion: The height of children with epilepsy is lower than that of normal chil dren, and the incidence of short stature is higher than the average level. There was no significant differ ence in the effect of epileptic discharge on the growth in different parts. The proportion of height below normal children was higher, and the incidence of short stature was also significantly higher in children with epileptic discharge 〉50%. It is suggested that epileptic discharge may have an influence on the height of children with epilepsy, and it is worthy of further research.
