阿奇霉素序贯疗法治疗小儿支原体肺炎的临床疗效及其安全性

Clinical effect and safety of azithromycin sequential therapy in treating pediatric mycoplasma pneumonia

目的探讨阿奇霉素序贯疗法治疗小儿支原体肺炎的临床疗效及其安全性。方法选取山东德州联合医院2015年4月—2017年4月收治的支原体肺炎患儿100例,采用随机数字表法分为对照组和观察组,各50例。对照组患儿采用红霉素序贯疗法,观察组患儿采用阿奇霉素序贯疗法。比较两组患儿症状改善时间(退热时间、咳嗽消失时间及肺部啰音消失时间)及治疗前后肺功能指标[第1秒用力呼气容积(FEV1)、用力肺活量(FVC)及FEV1/FVC],并观察患儿不良反应发生情况。结果观察组患儿退热时间、咳嗽消失时间、肺部啰音消失时间短于对照组(P<0.05)。治疗前,两组患儿FEV1、FVC及FEV1/FVC比较,差异无统计学意义(P>0.05);治疗后,观察组患儿FEV1、FVC及FEV1/FVC高于对照组(P<0.05)。观察组患儿不良反应发生率低于对照组(P<0.05)。结论阿奇霉素序贯疗法治疗小儿支原体肺炎的临床疗效确切,其可缩短患儿症状改善时间,有效改善肺功能,且安全性较高。

Objective To investigate the clinical effect and safety of azithromycin sequential therapy in treating pediatric mycoplasma pneumonia.Methods A total of 100 cases of mycoplasma pneumonia children were selected from April 2015 to April 2017 in Shandong Dezhou United Hospital,which were divided into control group and observation group according to random number table method,50 cases in each group.The control group was given erythromycin sequential therapy,and the observation group was treated with azithromycin sequential therapy.The improvement time of symptoms（fever clearance time,disappearance time of cough and disappearance time of the lungs）,lung function indexes（FEV1,FVC,FEV1/FVC） before and after treatment were compared between the two groups,and the incidence of adverse reactions were observed.ResultsThe fever clearance time,disappearance time of cough and disappearance time of the lungs in observation group were shorter than control group（P〈0.05）.Before treatment,no statistically significant differences of FEV1,FVC or FEV1/FVC was found between the two groups（P〈0.05）;after treatment,the observation group of FEV1, FVC, FEV1/FVC were higher than control group（P〈0.05）.The incidence rate of adverse reactions in observation group was lower than control group（P〈0.05）.Conclusion Azithromycin sequential therapy has an obviously clinical effect in treating pediatric mycoplasma pneumonia,it can shorten the symptoms improvement time,effectively improve lung function of children,and with higher safety.
