养肝益水颗粒对高血压病早期肾损害的治疗效果及对血压变异性的影响

Treatment Effect of Yanggan Yishui Granule on Early Renal Injury in Hypertension Patients and The Effect on Blood Pressure Variability

目的：探讨养肝益水颗粒对高血压病早期肾损害的治疗效果，及对患者血压变异性的影响。方法：回顾性分析2016年5月-一2017年6月在医院接受治疗的高血压早期肾损害患者的临床资料，根据其用药分为对照组和观察组，其中对照组给予常规药物治疗，观察组在此基础上给予养肝益水颗粒治疗，治疗时间为8周。观察两组患者治疗前后肾功能指标、炎症因子水平和血压变异性的差异，比较两组患者心血管事件发生率和不良反应发生率的差异。结果：两组患者治疗前肾功能指标无差别，治疗后，观察组mALB、β2-G水平低于对照组；两组患者治疗前炎症因子水平无差别，治疗后，观察组IL-6、TNF-α和CRP水平低于对照组；两组患者治疗前血压无差别，治疗后，观察组24h平均SBP和24h平均DBP水平低于对照组；两组患者治疗前血压变异性无差别，治疗后，观察组24 h SBP SD、24 h DBP SD、24 h SBP ARV和24 h DBP ARV低于对照组；两组患者在治疗期间均未出现明显的心脏和血管病变，且无明显肝功能损害和胃肠道反应。结论：养肝益水颗粒对高血压病早期肾损害患者有较好的治疗效果，可明显改善患者肾功能和血压变异性，具有良好的应用价值。

Objective ： To investigate the therapeutic effect of Yanggan Yishui Granule on renal damage in early stage of hypertension, and their effects on blood pressure variability. Methods ： The clinical data of hypertensive patients with ear- ly renal impairment treated in our hospital from May 2016 to June 2017 was retrospectively analyzed. The patients were divided into control group and observation group according to their medication. The control group was given routine drug treatment, and the observation group was given Yanggan Yishui Granule. The treatment time was 8 weeks. The differ- ences of renal function indexes, inflammatory factors and blood pressure variability between the two groups were observed before and after treatment, and the difference of cardiovascular events and adverse reactions between the two groups were compared. Results：There was no difference in renal function between the two groups before treatment. After treatment, the levels of mALB and beta 2 - MG in the observation group were lower than those in the control group. Two groups had no difference between the levels of proinflammatory cytokines. After treatment, the observation group＇ s IL - 6, TNF - ot and CRP levels were lower than those of the control group. There was no difference in blood pressure before treatment in the two groups. After treatment, the 24 h mean SBP and 24 h mean DBP in the observation group were lower than those in the control group, and the average DBP level was lower than that of the control group. There was no difference in blood pressure variability in the two groups before and after treatment. After treatment, 24 h SBP, SD, 24 h DBP, SD, 24 h SBP, ARV and 24 h DBP ARV in the observation group were lower than those in the control group. There were no obvious car- diae and vascular changes during the treatment period in the two groups, and there was no obvious liver damage or gastrointes- tinal reaction. Conclusion： Yanggan Yishui Granule has a good therapeutic effect for patients with early renal injury in hyper- tension patients, and can significantly improve renal function and blood pressure variability, with good application value.
