动态血糖管理模式在危重症患者血糖中的应用

Application of dynamic blood glucose control management mode in critically ill patients with stress hyperglycemia

目的探讨动态血糖管理模式在危重症患者血糖中的应用效果。方法将2017年1月-10月本院ICU收治的120例危重症患者按时间分段分为两组,每组分别60例。对照组危重症患者实施传统血糖监测管理,试验组危重症患者实施动态血糖管理模式。比较两组危重症患者住院期间血糖最高值、血糖最低值、血糖恢复时间及低血糖、高血糖发生情况。结果两组危重症患者血糖最低值比较,P>0.05,差异无统计学意义;两组危重症患者血糖最高值、血糖恢复时间比较,均P<0.05,差异有统计学意义,试验组患者血糖最高值和血糖恢复时间低于或短于对照组。两组危重症患者低血糖发生率、高血糖发生率比较,均P<0.01,差异有统计学意义,试验组患者低血糖、高血糖发生率均低于对照组。结论动态血糖管理模式在危重症患者血糖中的应用,能使危重患者血糖得到动态的、连续的监测,从而更加合理地调节治疗方案,稳定患者血糖水平,减少异常血糖的发生率,此方法值得临床推广应用。

Objective To explore the effect of application of dynamic blood glucose control management mode in critically ill patients with stress hyperglycemia. Methods A total of 120 critical patients admitted to intensive care unit from January to October in2017 divided into two groups according to their admission time, 60 in each group. The patients who used the traditional mode of blood glucose control were assigned as control group, and those who used the dynamic blood glucose control management mode as experimental group. The two groups were compared in terms of blood glucose level, blood glucose recovery time and the incidences of hypoglycemia and hyperglycemia. Results The two groups did not differ in the lowest blood glucose（P 〉0.05）. The highest blood glucose and the blood glucose recovery time in the experimental group were lower than those in control group（ P〈0.05）. The incidences of hypoglycemia and hyperglycemia in the experimental group were significantly lower than those in control group（ P〉 0.01）.Conclusion The dynamic blood glucose control management mode applied in critically ill patients with stress hyperglycemia is effective in controlling the stress hyperglycemia＇s glucose level, reducing the incidence of abnormal blood glucose, and maintaining a stable blood glucose at the target level.
