缩宫素联合鲜益母草胶囊及缩宫素联合五加生化胶囊治疗产后子宫复旧不良的临床疗效比较

Comparison of the therapeutic effects of oxytocin combined with fresh motherwort capsule and oxytocin combined with Wujia Shenghua capsule in the treatment of postpartum uterine malignancy

目的:探讨缩宫素联合鲜益母草胶囊及缩宫素联合五加生化胶囊治疗产后子宫复旧不良的临床疗效比较。方法:选取同济大学附属上海第十人民医院2015年2月至2017年2月收治的产后子宫复旧不良患者280例进行回顾性分析,按治疗方式不同分为3组,A组单独使用缩宫素治疗,B组使用缩宫素联合鲜益母草胶囊,C组使用缩宫素联合五加生化胶囊治疗。对比3组患者治疗有效率、各项临床症状和体征变化、产后阴道出血量情况。结果:治疗10d后,B、C组治疗总有效率高于A组(P<0.05),差异具有统计学意义。B组治疗总有效率与C组比较,差异无统计学意义(P>0.05)。A组阴道流血时间积分高于B组、C组(P<0.05),差异具有统计学意义。B组阴道流血时间积分与C组比较,差异无统计学意义(P>0.05)。A组产后5d子宫高度积分、15d B超三径积分高于B组、C组(P<0.05),差异具有统计学意义。C组产后5d子宫高度、15d B超三径高于B组(P<0.05)。A组、B组、C组宫缩痛时间积分比较,差异无统计学意义(P>0.05)。A组产后3d阴道出血量多于B组、C组(P<0.05),差异具有统计学意义。B组产后3d阴道出血量与C组比较,差异无统计学意义(P>0.05)。结论:缩宫素联合鲜益母草胶囊或五加生化胶囊治疗产后子宫复旧不良均具有显著效果,可提高治疗有效率,改善临床症状和体征,减少产后阴道出血量,值得临床应用。

Objective： To investigate the clinical efficacy of oxytocin combined with fresh motherwort capsule and oxytocin combined with Wujia Shenghua capsule in the treatment of postpartum uterine malignancy.Methods： 280 patients with postpartum uterine recurrent malignancies received from February 2015 to February2017 were retrospectively analyzed. According to the different treatment methods,they were divided into three groups. Group A was treated with oxytocin alone; group B received oxytocin combined with fresh motherwort capsule; group Cwas given oxytocin combined with Wujia Shenghua capsule treatment. The effective rate,changes in clinical symptoms and signs and postpartum hemorrhage volume were compared between the three groups. Results：At 10 days after treatment,the total effective rate in group B and group C was higher than that in group A,with statistically significant difference（ P 〈 0. 05）. There was no significant difference between group B and group C（ P 0. 05）. The time of vaginal bleeding in group A was longer than that in group B and group C,with statistically significant difference（ P 〈 0. 05）. There was no significant difference between group B and group C（ P 〉 0. 05）. The uterusheight integral of group A at 5 d postpartum and B ultrasound integral at 15 d postpartum in group A were higher than those in group B and group C,and those of group C were higher than group B,with statistically significant difference（ P 〈 0. 05）. There was no significant difference in uterine contraction time integral among the threegroups（ P 〉0. 05）. The postpartum hemorrhageamount in group A was larger than group B and group C,with statistically significant difference（ P 〈 0. 05）,while difference between group B and group C was not statistically significant（ P 〉0. 05）. Conclusions： Oxytocin combined with fresh motherwort capsule or Wujia Shenghua capsuleshave significant effect in the treatment of postpartum uterine malignancy,which can improve the treatmentefficiency,clinical symptoms and signs and reduce postpartum hemorrhage,hereby worthy of clinical application.
