米索前列醇联合硫酸镁治疗妊娠高血压综合征产后出血的疗效分析

Effect of Misoprostol Combined with Magnesium Sulfate on Postpartum Hemorrhage in Pregnancy-induced Hypertension

目的评价米索前列醇、硫酸镁联合应用治疗妊娠高血压综合征产后出血的临床疗效。方法方便选取该院于2015年12月—2017年9月收治的84例妊娠高血压综合征患者,随机分为对照组和联合组,每组42例,分别予以两组患者硫酸镁治疗和米索前列醇、硫酸镁联合治疗,比较两组患者治疗前、后的血压状况、产后2 h、24 h出血量和药物不良反应情况。结果两组患者治疗前的MAP(平均动脉压)比较差异无统计学意义(P>0.05),联合组治疗后的MAP为(85.27±4.92)mm Hg,低于对照组的(91.60±5.33)mm Hg(t=3.903,P=0.000<0.05)。联合组产后2 h、24 h的出血量分别为(179.26±36.05)m L、(240.25±37.18)m L,少于对照组的(212.10±35.37)m L、(317.49±43.08)m L(t=3.910,6.070,P=0.006,0.000<0.05)。两组药物不良反应发生率分别为7.14%、4.16%,差异无统计学意义(χ~2=0.213、P=0.645>0.05)。结论米索前列醇、硫酸镁联合应用治疗妊娠高血压综合征产后出血,能够控制患者血压,减少患者产后出血量,疗效可靠,不良反应少。

Objective This paper tries to evaluate the clinical efficacy of misoprostol and magnesium sulfate combined with postpartum hemorrhage in pregnancy induced hypertension. Methods 84 cases of pregnancy-induced hypertension syndrome admitted to the hospital from December 2015 to September 2017 were convenient selected and randomly divided into the control group and the combined group. There were 42 cases in each group. Each group received magnesium sulfate treatment and misoprostol, magnesium sulphate combined treatment, compared the blood pressure before and after treatment in the two groups of patients, postpartum 2 h, 24 h bleeding and adverse drug reactions. Results There was no significant difference in MAP（mean arterial pressure） before treatment between the two groups（P〉0.05）. The MAP after combined treatment was（85.27±4.92） mmHg, which was lower than that of the control group（91.60±5.33）mmHg（ t=3.903, P=0.000〈0.05）.The blood loss of the combined group at 2 h and 24 h postpartum was（179.26±36.05）mL and（240.25±37.18）mL, respectively, less than that of the control group（212.10±35.37）mL and（317.49±43.08）mL（t=3.910, 6.070, P=0.006,0.000〈0.05）. The incidence of adverse drug reactions in the two groups was 7.14% and 4.16%, respectively, and the difference was not statistically significant（χ^2=0.213, P=0.645〉0.05）. Conclusion The combination of misoprostol and magnesium sulfate in the treatment of postpartum hemorrhage in pregnancy-induced hypertension syndrome can control the blood pressure of the patient and reduce the postpartum hemorrhage. The curative effect is reliable and the adverse reactions are fewer.
