血清降钙素原(PCT)检测在脓毒症诊断上的应用价值

摘要 目的探讨血清降钙素原(PCT)检测在脓毒症诊断上的应用价值。方法方便选取该院2015年11月—2016年11月收治的患者120例进行该次研究,有72例患者为脓毒症患者,48例患者为非脓毒症患者,根据脓毒症患者的严重程度,将其分为观察A组(46例)与观察B组(26例)。所有患者在入院时以及住院后的第3、5天实施血清PCT检测,统计分析患者的急性生理功能与慢性健康状况评分系统Ⅱ(APACHEⅡ)的评定结果以及序贯器官衰竭估计评分的评定结果。结果入院时,观察组患者的APACHEⅡ评分(17.54±3.95)分、SOFA评分(6.37±2.04)分以及血清PCT水平(3.15±0.83)μg/L均明显高于对照组的(2.47±0.41)分、(0.57±0.08)分以及(0.39±0.17)μg/L,差异有统计学意义(P<0.05);与入院时相比,观察A组第3天(1.37±0.45)μg/L与第5天(1.25±0.41)μg/L的血清PCT明显下降,观察B组(7.21±2.13)μg/L、(8.49±2.13)μg/L明显上升;观察A组、观察B组入院时、入院第3天、入院第5天的血清PCT均比对照组要高,差异有统计学意义(P<0.05);观察B组的血清PCT水平在入院时、入院第3天、入院第5天均明显高于观察A组,差异有统计学意义(P<0.05)。结论血清降钙素原(PCT)检测对于脓毒症诊断具有较好的应用效果,可以及时发现和判断患者的患病情况,及时采取有效的治疗方法,值得临床推广。 Objective This paper tries to explore the value of serum procalcitonin(PCT) test in the diagnosis of sepsis.Methods 120 patients who were admitted to the hospital from November 2015 to November 2016 were convenient selected in this study. There were 72 patients with sepsis and 48 patients with non-sepsis, according to the severity of sepsis patients. They were divided into observation group A(46 cases) and observation group B(26 cases). All patients underwent serum PCT testing at admission and on the 3 rd and 5 th day after hospitalization. Statistical analysis was performed to assess the patient's acute physical function and chronic health status scoring system II(APACHE II) and the sequential assessment of organ failure, and the results were evaluated. Results At the time of admission, patients in the observation group had significantly higher APACHE II scores(17.54±3.95)points, SOFA scores(6.37±2.04)points, and serum PCT levels(3.15±0.83)μg/L than those in the control group(2.47±0.41)points and(0.57±0.08)points,(0.39±0.17)μg/L, and the difference was statistically significant(P〈0.05). Compared with the time of admission, serum PCT on the 3 d(1.37±0.45)μg/L and 5 d(1.25±0.41)μg/L in group A was significantly decreased. Observation group B(7.21±2.13)μg/L,(8.49±2.13)μg/L increased significantly; observation group A, observation group B admission, hospital admission on the third day, hospital admission on the fifth day serum PCT were higher than the control group, and the difference was statistically significant(P〈0.05); The serum PCT level in the observation group B was significantly higher than that in the observation group A at the time of admission,admission on the 3 rd day, and admission on the 5 th day, and the difference was statistically significant(P〈0.05). Conclusion The serum procalcitonin(PCT) test has a good application effect on the diagnosis of sepsis. It can detect and determine the patient's condition in time and take effective treatment methods in time. It is worthy of clinical promotion.
