益气固表丸对慢性阻塞性肺疾病稳定期吸烟患者外周血Th17/Treg免疫平衡的影响

Effect of Yiqi Gubiao pill on Th17/Treg immune balance in peripheral blood of smoking patients with chronic obstructive pulmonary disease in stable stage

目的观察益气固表丸对慢性阻塞性肺疾病(COPD)稳定期吸烟患者外周血辅助性T细胞17(Th17)/调节性T细胞(Treg)免疫平衡的影响。方法将172例COPD稳定期吸烟患者随机分为2组。对照组86例采用常规西医治疗;治疗组86例在对照组治疗基础上联合益气固表丸治疗。2组均治疗3个月。比较2组治疗前后外周血Th17/Treg比例,血清炎症因子[白细胞介素8(IL-8)、IL-10、肿瘤坏死因子α(TNF-α)]、肺功能指标[用力肺活量(FVC)、第1 s用力呼气容积(FEV1)、FEV1/FVC]、6 min步行距离、改良呼吸困难指数(m MRC)、COPD评估测试(CAT)评分。结果治疗后2组Th17、Treg、Th17/Treg与本组治疗前比较均降低(P<0.05),且治疗组Th17、Treg、Th17/Treg均低于对照组(P<0.05)。治疗后2组IL-8、TNF-α水平均降低(P<0.05),IL-10水平升高(P<0.05),且治疗组IL-8、TNF-α水平均低于对照组(P<0.05),IL-10水平高于对照组(P<0.05)。治疗后2组FVC、FEV1、FEV1/FVC与本组治疗前比较均升高(P<0.05),且治疗组FVC、FEV1、FEV1/FVC均高于对照组(P<0.05)。治疗后2组6 min步行距离增加(P<0.05),m MRC、CAT评分降低(P<0.05),且治疗组6 min步行距离高于对照组(P<0.05),m MRC、CAT评分低于对照组(P<0.05)。结论益气固表丸能维持COPD稳定期吸烟患者外周血Th17/Treg免疫平衡,改善患者临床症状。

Objective To observe the effects of Yiqi Gubiao pill on Th17/Treg immune Balance in peripheral blood of smoking patients with chronic obstructive pulmonary disase（COPD）in stable stage.Methods172smoking patients with stable COPD were randomly divided into 2 groups.86 patients in control group were treated by conventional western medicine.86 patients in treatment group were treated by Yiqi Gubiao pill on the basis of the control group.Both groups were treated for 3 months.The ratio of Th17/Treg in peripheral blood of two groups,serum inflammatory factors[interleukin-8（IL-8）,IL-10,tumor necrosis factor alpha（TNF-α）],lung function index[forced vital capacity（FVC）,1 s forced expiratory volume（FEV1）FVC],6 min walking distance,modified dyspnea index（m MRC）,COPD assessment test（CAT）score were compared.Results The Th17,Treg,Th17/Treg in the two groups were lower than those before treatment（P〈0. 05）,and Th17,Treg and Th17/Treg in the treatment group were lower than those in the control group（P〈0. 05）.The levels of IL-8 and TNF-αin both groups were decreased than those before treatment（P〈0. 05）,and IL-10 levels were increased（P〈0. 05）.The levels of IL-8 and TNF-αin the treatment group were lower than those in the control group（P〈0. 05）,and IL-10 level was higher than the control group（P〈0. 05）.After FVC were increased in both groups（P〈0. 05）,andFVC in the treatment group were higher than those in the control group（P〈0. 05）.After treatment,the 6-minute walking distance of the two groups increased（P〈0. 05）,and the m MRC and CAT scores decreased（P〈0. 05）.The walking distance of the treatment group was higher than that of the control group（P〈0. 05）,and the m MRC and CAT scores were lower than the control group（P〈0. 05）.ConclusionYiqi Gubiao pill can maintain the immune balance of Th17/Treg in peripheral blood of patients with stable COPD and improve clinical symptoms.
