刃针联合柴胡桂枝干姜汤治疗肝郁脾虚型围绝经期抑郁症临床观察

Clinical observation of blade needle combined with Chaihu-Guizhi-Ganjiang decoction on peri-menopausal depression with liver stagnation and spleen deficiency syndrome

目的观察刃针联合柴胡桂枝干姜汤治疗肝郁脾虚型围绝经期抑郁症的临床疗效。方法将60例肝郁脾虚型围绝经期抑郁症患者按照随机数字表法分为2组。治疗组30例予刃针联合柴胡桂枝干姜汤治疗;对照组30例单纯应用柴胡桂枝干姜汤治疗。2组均4周为1个疗程,治疗2个疗程。比较2组疗效;比较2组治疗前后改良围绝经期Kupperman评分、汉密尔顿抑郁量表(HAMD)评分变化;测定患者治疗前后性激素水平[雌二醇(E2)、卵泡刺激素(FSH)、黄体生成激素(LH)]变化。结果治疗组愈显率93.33%、总有效率96.67%,对照组愈显率66.66%、总有效率90.00%,治疗组疗效优于对照组(P<0.05)。治疗后2组Kupperman评分、HAMD评分均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。治疗后治疗组E2较本组治疗前升高(P<0.05),FSH、LH均较本组治疗前降低(P<0.05);对照组治疗后E2较本组治疗前升高(P<0.05);治疗后治疗组E2高于对照组(P<0.05),FSH、LH均低于对照组(P<0.05)。结论刃针联合柴胡桂枝干姜汤治疗肝郁脾虚型围绝经期抑郁症效果显著,能改善患者围绝经期症状,降低抑郁程度,提高E2水平,优于单纯应用柴胡桂枝干姜汤治疗。

Objective To observe the clinical effects of blade needle combined with Chaihu-Guizhi-Ganjiang decoction on peri-menopausal depression with liver stagnation and spleen deficiency syndrome.Methods60patients with peri-menopausal depression with liver stagnation and spleen deficiency syndrome were divided into two groups according to the random number table method.30 cases in treatment group were treated by blade needle combined with Chaihu-Guizhi-Ganjiang decoction,and 30 cases in control group were treated by Chaihu-GuizhiGanjiang decoction.Both groups were treated 4 weeks for 1 course of treatment and treated for 2 courses.The efficacy of the two groups was compared.The changes of peri-menopausal Kupperman score and Hamilton Depression Scale（HAMD）score were compared before and after treatment.The hormone levels including Estradiol（E2）,follicle stimulating hormone（FSH）,luteal production Hormone（LH）before and after treatment were determined.Results The markedly effective rate was 93.33%and the total effective rate was 96.67%in treatment group.The markedly effective rate was 66.66%and the total effective rate was 90.00%in control group.The treatment group was better than the control group（P〈0. 05）.Kupperman scores and HAMD scores in the two groups were lower than those in the group before treatment（P〈0. 05）,and the treatment group was more significant（P〈0. 05）.E2in treatment group was higher than that in the group before treatment（P〈0. 05）,FSH and LH in treatment group were lower than those in the group before treatment（P〈0. 05）.E2in control group was higher than that in the group before treatment（P〈0. 05）.Afterhigher in treatment group than in the control group（P〈0. 05）,and FSH and LH in treatment group were lower than the control group（P〈0. 05）.ConclusionThe combination of blade needle and Chaihu-Guizhi-Ganjiang decoction in the treatment of peri-menopausal depression of liver stagnation and spleen deficiency can effectively improve the perimenopausal symptoms,reduce the degree of depression and increase the level ofis better than the simple application of Chaihu-Guizhi-Ganjiang-Decoction.
