自拟益气化瘀汤联合左甲状腺素钠治疗与分化型甲状腺癌术后患者临床研究

Clinical Research on Treating Differentiated Thyroid Cancer Postoperative Patients with Yiqi Huayu Decoction Combined Levothyroxine Sodium

目的：观察用自拟益气化瘀汤联合左甲状腺素钠治疗分化型甲状腺癌术后患者的临床效果。方法：将2013年2月—2015年2月本院收治的136例分化型甲状腺癌术后患者按照随机数字表法分为对照组和观察组患者各68例。对照组患者术后给予左甲状腺素钠治疗;观察组患者则联合给予自拟益气化瘀汤内服,6个月为1个疗程,两组均连续用药2个疗程。比较两组治疗后的临床效果,观察甲状腺功能变化及术后不同时间段患者生活质量的改善情况。结果：观察组的临床疗效91.2%较对照组75%明显升高（χ~2=6.332,P=0.021〈0.05）;观察组患者术后甲状腺功能改善优于对照组患者,其血清游离甲状腺素（FT3）、游离三碘甲状腺原氨酸（FT4）显著高于对照组患者,促甲状腺激素（TSH）显著低于对照组患者（P〈0.05）;观察组患者术后经治疗6个月、1年后QLQ-C30生活质量相关的角色、躯体、认知、感情、社会等5个功能领的评分与对照组相比显著升高（P〈0.05）。结论：自拟益气化瘀汤联合左甲状腺素钠治疗分化型甲状腺癌术后患者疗效确切,能显著改善患者的甲状腺功能,提高患者的生活质量,值得临床推广运用。

Objective：To investigate the clinical efficacy of treating differentiated thyroid cancer postoperative patients with Yiqi Huayu Decoction combined levothyroxine sodium. Methods：A total of 136 patients were randomly divided into a control group of68 cases and an observation group of 68 cases. Patients in the control group were treated with levothyroxine sodium,while patients in the observation group were treated with Yiqi Huayu Decoction combined with levothyroxine sodium,six months for a course,totally 2 courses for the treatment. The research was aimed at comparing the clinical efficacy,observing the thyroid function changes and the improvement of the quality of life in patients with postoperative different time periods. Results：The efficiency of observation group was 91. 2%,while the efficiency of control group was 75%. The efficiency of observation group was higher compared with that of the control group（ χ2= 6. 332,P = 0. 021 0. 05）. The thyroid function changes of observation group were better than those of the control group. The levels of FT3 and FT4 were higher than those of the control group,but the level of TSH was lower than that of the control group（ P〈0. 05）. The patients＇ five function scores of quality of life related the physical,cognitive,emotional and social conditions were higher than those of the control group（ P〈0. 05）. Conclusion：The therapy has the satisfactory clinical effect,and it can effectively improve patients＇ thyroid function and improve the patients＇ quality of life,worthy of wide application in the clinical practice.
