步行阶梯训练对老年糖尿病周围神经病变患者步态调控的影响

Effect of Step-walking Training on Gait Control in the Elderly with Diabetic Peripheral Neuropathy

目的评价步行阶梯训练对老年糖尿病周围神经病变(diabetic peripheral neuropathy,DPN)患者步态调控的影响。方法采用自身前后对照设计,对上海市某社区卫生服务中心招募的35例老年DPN患者进行3个月步行阶梯训练,比较患者训练前后的步态调控能力。结果训练后患者的足底压力中心轨迹曲线异常比例、全足压力变化曲线异常比例及全足平衡性曲线异常比例减少(P<0.05)。结论步行阶梯训练有利于纠正老年DPN患者异常行走步态,增加姿势稳定性。借助步行阶梯训练装置可增加运动训练的便利性、有效性和安全性。

Objective To evaluate the effect of step-walking training on gaite control in the elderly with diabetic peripheral neuropathy（DPN）. Methods One-group pretest-posttest design was conducted and 35 elderly with diabetic peripheral neuropathy from a community health service center in Shanghai were enrolled for the step-walking training for 3 months; a comparison was made between the gait control competence of the elderly before and after training. Results After training, the proportion of abnormal centre of pressure（COP）, total force line and full balance line of the elderly decreased significantly（P0.05）.Conclusions Step-walking training is of help in correcting the abnormal walking gait and increasing the postural stability of the elderly with DPN; the step-walking training devices can promote the convenience, effectiveness and safety of the training.
