自动痔疮套扎术结合剪口结扎术治疗混合痔的效果

Effect of automatic ligation of hemorrhoids combined with scissors ligation in the treatment of mixed hemorrhoids

目的探讨自动痔疮套扎术(RPH)结合剪口结扎术治疗混合痔的效果。方法选取我院2015年1月~2017年3月收治的200例混合痔患者,采用随机数字表法将其分为对照组、联合手术组,每组100例。对照组应用剪口结扎术治疗,联合手术组应用RPH结合剪口结扎术治疗。比较两组混合痔治疗效果;术后出院时间、伤口平均愈合时间;手术前后患者肛管静息压、舒张压、最大收缩压,并发症总发生率。结果联合手术组患者的治疗总有效率高于对照组,差异有统计学意义(P<0.05);术前,两组患者肛管静息压、舒张压、最大收缩压的比较,差异均无统计学意义(P>0.05);术后,联合手术组患者的肛管静息压、舒张压、最大收缩压均低于对照组,差异均有统计学意义(P<0.05);联合手术组患者的术后出院时间、伤口平均愈合时间均短于对照组,差异均有统计学意义(P<0.05);联合手术组患者的并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论 RPH结合剪口结扎术治疗混合痔的效果确切,可有效加速伤口愈合,减少并发症的发生,改善肛管舒张压、收缩压等情况,有利于缩短住院时间和更好改善患者生存质量,值得推广。

Objective To investigate the efficacy of automatic ligation of hemorrhoids（RPH） combined with incision ligation in the treatment of mixed hemorrhoids. Methods A total of 200 mixed hemorrhoids patients admitted to our hospital from January 2015 to March 2017 were selected and divided into control group and combined operation group by random number table method, with 100 cases in each group. The control group was treated with scissors ligation,while the combined operation group was treated with RPH combined with scissors ligation. The curative effect of the two groups of mixed hemorrhoids was compared. Postoperative discharge time and average wound healing time. Before and after the operation, the patient′s anal canal rest pressure, diastolic pressure, maximum systolic pressure, and the total incidence of complications. Results The total effective rate of the combined operation group was higher than that of the control group, the differences were statistically significant（P〈0.05）. Before surgery, there was no statistical difference between the two groups（P〈0.05）, in the static pressure, diastolic pressure and maximum systolic pressure of anal canal. After the operation, the anal canal rest pressure, diastolic pressure and maximum systolic pressure in the combined operation group were lower than those in the control group, the differences were statistically significant（P〈0.05）.The postoperative discharge time and average wound healing time in the combined operation group were shorter than those in the control group the differences were statistically significant（P〈0.05）. The total incidence of complications in the combined operation group was lower than that in the control group, and the difference was statistically significant（P〈0.05）. Conclusion RPH combined with incision ligation is effective in treating mixed hemorrhoids. It can effectively accelerate wound healing,reduce complications, improve anal diastolic pressure and systolic pressure. It is beneficial to shorten hospitalization time and better improve the quality of life of patients. It is worth promoting.
