氟伐他汀对冠心病心力衰竭患者心脏结构功能及循环指标影响研究

Effects of fluvastatin on cardiac structure,function and circulation in patients with coronary heart disease and heart failure

目的研究氟伐他汀对冠心病心力衰竭患者心脏结构、功能及循环指标影响。方法选取我院2015年10月到2017年4月收治的冠心病心力衰竭患者82例作为研究对象,采用随机数字法将其分为对照组(41例)和观察组(41例)。对照组患者接受常规治疗,同时服用阿罗洛尔,在此基础上,观察组患者服用氟伐他汀治疗。比较2组患者的治疗总有效率和不良反应发生率,同时比较治疗前后的心脏结构和功能指标、循环指标及炎症因子水平。结果观察组的治疗总有效率(93%)明显高于对照组(73%)(χ2=5.91,P=0.00);治疗后,观察组的左心室舒张期末径(LVEDD)值明显低于对照组(P<0.01),观察组的左心室射血分数(LVEF)、心脏指数(CI)及每搏输出量(SV)值均明显高于对照组(P<0.01),观察组的LVEDD值明显低于对照组(P<0.01),观察组的肺动脉楔压(PAWP)、体循环阻力(SVR)及肺循环阻力(PVR)值均明显低于对照组(P<0.01),观察组的平均动脉压(MAP)值明显高于对照组(P<0.01),观察组的血清肿瘤坏死因子(TNF)-α、白细胞介素(IL)-6及高敏-C反应蛋白(hs-CRP)水平均明显低于对照组(P<0.01)。结论氟伐他汀联合阿罗洛尔用于冠心病心力衰竭治疗的疗效明显,能明显改善患者的心脏结构及功能,促进机体循环正常,减轻炎症反应程度,且药物不良反应少,值得在临床推广。

Objective To study the effect of fluvastatin on cardiac structure,function and circulation in patients with coronary heart disease and heart failure.Methods The subjects were 82 patients with coronary heart disease in our hospital from October 2015 to April 2017.They were randomly divided into control group and observation group,41 cases in each group.The patients in the control group received routine treatment and took alrolol.On this basis,the patients in the observation group were treated with fluvastatin.Total effective rate and the incidence of adverse reactions were compared between the two groups,and the cardiac structure and function indexes,circulatory indexes and inflammatory factors before and after treatment were compared.Results The total effective rate（93%）in the observation group was significantly higher than that of the control group（73%）（χ^2=5.91,P〈0.01）.After treatment,the LVEDD value of the observation group was significantly lower than that of the control group（P〈0.01）.The LVEF,CI and SV values were significantly higher than those of the control group（P〈0.01）.The levels of serum TNF-α,IL-6 and hs-CRP were significantly lower than those of the control group（P〈0.01）,and the MAP values were significantly lower than that of the control group（P〈0.01）.Conclusion Fluvastatin is effective in the treatment of patients with coronary heart disease and heart failure.It can effectively promote the recovery of heart structure and function,improve the circulation of the body,inhibit the inflammation and it is of high safety.It is worthy to be popularized in clinical practice.
