伤椎置入螺钉长度对胸腰椎骨折复位的影响

The effect of screw length of the injured vertebra on fracture reduction of thoracolumbar fracture

目的探讨在胸腰椎骨折手术治疗中伤椎置入螺钉长度对骨折复位的影响。方法将35例胸腰椎骨折患者按伤椎置入螺钉长度分为两组:研究组18例为伤椎椎弓根置入标准螺钉(45 mm),对照组17例为伤椎椎弓根置入短螺钉(35 mm)。两组患者均采用后路手术,比较两组术后椎体前缘高度、后凸Cobb角、椎间隙高度变化及术后12个月维持情况。结果患者均获得随访,时间12～30个月。两组椎体前缘高度、椎间隙高度和后凸Cobb角术后均较术前明显改善(P<0.05)。术后12个月伤椎骨折复位的维持情况研究组明显优于对照组(P<0.05)。结论伤椎置钉对胸腰椎骨折具有良好的复位效果,伤椎标准螺钉对骨折复位及维持复位的效果更好。

Objective To investigate the effect of injured vertebra screw length on fracture reduction. Methods The 35 patients with thoracolumbar fracture were divided into two groups according to the injured vertebra screw length： the research group of 18 cases with injured vertebra placeing standard pedicle screw（ 45 mm）,the control group of 17 cases with injured vertebral pedicle in short screw（ 35 mm）. All patients were treated with posterior surgery,two groups were compared,including preoperative and postoperative anterior vertebral height,kyphosis Cobb angle,and intervertebral space height change and postoperative 1 year maintenance. Results All patients were followed up for 12 ～ 30 months. Compared with the preoperative,the anterior vertebral height,the height of the intervertebral space and the Cobb angle at the postoperative were significantly improved（ P 0. 05）. At 12 months after surgery,the maintenance of injured vertebra fracture reduction in the research group was significantly better than the control group（ P 0. 05）. Conclusions Injured vertebral pedicle screw fixation has good reduction effect on thoracolumbar fracture,the standard screw is better for fracture reduction and maintenance of reduction.
