摘要
目的 探讨神经导航引导下软通道微创穿刺引流术对老年高血压性脑出血疗效及安全性.方法 选取自2014年7月至2017年03月在我院接受神经导航引导下软通道微创穿刺引流术治疗的20例老年高血压性脑出血患者作为实验组,选取同时期接受小骨窗开颅手术的25例高血压脑出血患者作为对照组.对2组患者手术时间、术中出血量、住院时间,术后NIHSS评分、临床疗效及术后并发症进行比较.结果 实验组较对照组在手术时间、术中出血量、住院时间有明显优势,差异有统计学意义(P<0.05),实验组术后NIHSS评分及临床疗效较对照组好,差异有统计学意义(P<0.05),实验组术后并发症明显较对照组少,差异有统计学意义(P<0.05).结论 在手术时间、出血量、住院时间、术后神经功能恢复和日常生活能力改善方面、术后并发症,神经导航软通道微创穿刺引流术较小骨窗手术具有显著优势.
Objective To investigate the effect and safety of soft channel minimally invasive puncture and drainage guided by neuronavigation on elderly hypertensive cerebral hemorrhage.Methods Twenty elderly patients with hypertensive intracerebral hemorrhage,who underwent soft channel minimally invasive puncture and drainage guided by neuronavigation between July 2014 and March 2017 in our hospital,were included in the study group.A contemporary cohort of 25 patients with hypertensive cerebral hemorrhage,who underwent craniotomy were included in the control group.The operative time,intraoperative blood loss,length of hospital stay,postoperative NIHSS score,clinical efficacy and postoperative complications were compared between the two groups.Results The operative time,intraoperative blood loss,and length of hospital stay in the study group showed significant advantages compared with those in the control group,and there were statistically significant differences (P〈0.05).The postoperative NIHSS score and clinical efficacy in the study group were better than those in the control group,and there were statistically significant differences (P〈0.05).The postoperative complications in the study group were significantly less than those in the control group,and there were statistically significant differences (P〈0.05).Conclusion The soft channel minimally invasive puncture and drainage guided by neuronavigation appears to have evident advantages on the operative time,blood loss,length of hospital stay,postoperative recovery of neurological function and improvement of daily living ability,and postoperative complications.
