80例卒中后癫痫的临床特点及危险因素分析

目的探讨卒中后癫痫(的临床特点及危险因素。方法 PSE)选取80例PSE患者作为观察组,80例脑卒中无PSE患者作为对照组。分析PSE的临床特点,并采用多因素logistic回归分析独立危险因素。结果观察组早发型PSE和迟发型PSE分别占65.0%和35.0%。观察组早发型PSE的单纯部分发作发病率高于迟发型PSE,全身性发作发病率明显低于迟发型PSE(均P<0.05)。观察组的肺部感染、病灶累及皮质、卒中史、梗死灶最大截面积直径≥4 cm和NIHSS评分≥12分者所占比例均高于对照组(P。结论 <0.01)PSE中以早发型常见,早发型PSE以单纯部分发作常见,迟发型PSE以全身性发作常见。肺部感染、病灶累及皮质、卒中史、梗死灶最大截面积的直径≥4 cm和NIHSS评分≥12分是PSE的独立危险因素。

Objective To investigate the clinical characteristics and risk factors of post-stroke epilepsy（PSE）. Methods Eighty cases of PSE were selected as the Observation Group while 80 cases of stroke without PSE were selected as the Control Group. The clinical characteristics and risk factors of PSE were statistically analyzed. Multiple factor Logistic regression was used to analyze independent risk factors. Results In the Observation Group, those with early-onset PSE and late-onset PSE accounted for 65%（52 cases） and 35%（28 cases）, those with early-onset PSE had the incidence of the simple partial seizure higher than those with late-onset PSE and the incidence of systematic seizure significantly lower than those with late-onset PSE（P〈0.05 for all）. In the Observation Group, the proportion of the patients with pulmonary infection, lesions involving the cortex, history of stroke, diameter of the largest cross-sectional area of infarction ≥4 cm and NIHSS score≥12 higher than that of Control Group（P〈0.01）. Conclusions Early-onset PSE is common. Simple partial seizures are more common in the early-onset PSE while systemic seizures are more common in the late-onset PSE. Pulmonary infection, lesions involving the cortex, history of stroke, diameter of the largest cross-sectional area of infarction ≥4 cm and NIHSS score≥12 are independent risk factors for PSE.
