摘要
目的探讨胰腺实性假乳头状瘤动态三期增强及^(18)F-FDG PET/CT显像的影像学特点。方法分析广东省人民医院9例经手术病理证实的胰腺实性假乳头状瘤的临床资料及^(18)F-FDG PET/CT和同机MSCT动态三期增强影像资料。结果 8例行^(18)F-FDG PET/CT和同机MSCT动态三期增强检查,1例行胰腺局部双时相^(18)F-FDG PET/CT检查。三期增强扫描SPTP实性成分有渐进性强化的特点,并且强化程度始终低于瘤周正常胰腺组织,瘤体增强程度与瘤周正常胰腺组织增强程度差异有统计学意义;三期增强比单期增强扫描提供更多的解剖信息。^(18)F-FDG PET/CT显像中大部分呈糖代谢增高的表现。结论动态三期增强结合^(18)F-FDG PET/CT显像可以清晰地显示肿瘤形态、位置、与周围血管脏器的关系以及代谢情况,对于本病的诊断以及术前评估有肯定的价值。
bjective To investigate the imaging features of triple-phase contrast enhanced CT and ^(18)F-FDG PET/CT imaging of solid-pseudopapillary tumor of pancreas. Methods The clinical data of 9 cases of solid pseudopapillary tumor of the pancreas confirmed by operation and pathology in Guangdong General Hospital, and the ^(18)F-FDG PET/CT and the triple-phase contrast enhanced MSCT imaging data of the same machine were analyzed retrospectively. Results 8 cases underwent the ^(18)F-FDG PET/CT and the triple-phase contrast enhanced MSCT while 1 case under went local dual-phase ^(18)F-FDG PET/CT. The triple-phase contrast enhanced CT showed that the solid component of SPTP had the characteristics of progressive enhancement, and the degree of enhancement was always lower than that of normal pancreas tissue around the tumor. There was a significant difference between the enhancement degree of tumor and that of normal pancreatic tissue. The triple-phase contrast enhanced scan offered more anatomical information than the single phase contrast enhanced scan. Most of ^(18)F-FDG PET/CT imaging showed increased glucose metabolism. Conclusion The triple-phase contrast enhanced CT combined with ^(18)F-FDG PET imaging can clearly display the tumor morphology, location, relationship with peripheral vessels and organs as well as metabolism, and can be of positive value for the diagnosis and preoperative evaluation of the disease.
