腋后缘线“W”形切口与腋中线“Z”形切口微创治疗腋臭疗效观察

Following-up of Curative Effect of Minimally Invasive Treatment on Bromhidrosis by W-shaped Incision along Posterior Axillary Line and Z-shaped Incision along Midaxillary line

目的比较腋后缘线"W"形切口法与腋中线"Z"形切口法微创治疗腋臭的临床效果。方法选择2015年7月至2017年7月在我科就诊的腋臭患者,对60例患者应用腋后缘线"W"形切口与60例患者应用腋中线"Z"形切口的临床效果进行比较分析。结果腋后缘线"W"形切口法患者的有效率98.3%,腋中线"Z"形切口法患者的有效率96.7%,差异无统计学意义(P>0.05);腋中线"Z"形切口法的术后并发症发生率多于腋后缘线"W"形切口法术后并发症发生率(P<0.05)。结论腋后缘线"W"形切口法具有愈合快,术后发生皮瓣坏死、伤口裂开、脂肪液化引起的愈合不良风险小,术后形成的瘢痕不明显,是目前临床治疗腋臭的较好方法。

Objective Comparation of clinical effect on Bromhidrosis by W-shaped incision along posterior axillary line with Z-shaped incision along midaxillary line. Methods By slecting patients in treatment at Department of Dermatology from July, 2015 to July, 2017,,Clinical effects on bromhidrosis of 60 patients by W-shaped Incision along Posterior Axillary Line and by Z-shaped Incision along Midaxillary line are compared and analyzed. Results 98.3% and 96.7% effective rate are obtained repsecitively by W-shaped incision along posterior axillary line and by Z-shaped Incision along Midaxillary line, and the difference between them is no statistical significance. But, incidence rate of postoperative complications resulted by Z-shaped incision along midaxillary line is higher than that caused by W-shaped Incision along Posterior Axillary Line（P〈0.05）. Conclustion after surgery, W-shaped incision along posterior axillary line has advantages of quick healing, lower risks of necrosis of skin flap and wound dehiscence and poor healing caused by fat necrosis as well as inapparent scar, therefore it is a preferable method to treat bromhidrosis for the time being.
