自体鼻中隔软骨联合耳软骨在鼻尖整形术中移植填充效果观察

Effects of application of autologous nasal septal cartilage and ear cartilage in the transplantation and filling of nasal tip plasty

目的探析自体鼻中隔软骨及耳软骨在鼻尖整形术中移植填充的效果。方法研究收集2015年6月至2017年6月于我院进行鼻尖整形的患者80例,把选入对象随机分为自体组与膨体组各40例,自体组使用自体鼻中隔软骨及耳软骨进行鼻尖填充,膨体组应用由聚四氟乙烯制成膨体假体进行鼻尖部填充,比较两组整形效果。结果手术后,两组鼻长、鼻尖高度、鼻尖角、鼻尖距均较治疗前明显改善(P<0.05),且自体组鼻长、鼻尖高度、鼻尖角、鼻尖距改善水平与膨体组比较差异无统计学意义(P>0.05);两组鼻尖高度、鼻尖角、鼻尖距、鼻尖突出度、鼻唇角美学达标率比较差异无统计学意义(P>0.05);两组医生满意度、患者满意度及第三者满意度比较差异无统计学意义(P>0.05);手术后1年内,自体组并发症发生率0.00%,显著低于对照组的15.00%(P<0.05)。结论采用自体鼻中隔软骨及耳软骨进行鼻尖整形,其整形效果与采用膨体填充相当,但自体组并发症更少,安全性较高。

Objective To explore the effects of autologous nasal septal cartilage and ear cartilage in the transplantation and filling of nasal tip plasty. Methods 80 cases of patients treated with nasal tip plasty in our hospital from June 2015 to June 2017 were collected for the study and were randomly divided into autologous group and bulk group（each n=40）. Autologous group was given autologous nasal septal cartilage and ear cartilage for nasal tip filling, and bulk group was given bulk prosthesis made by polytetrafluoroethylene for nasal tip filling. The plastic effect was compared between the two groups. Results After operation, the nasal length, nose tip height, nasorostralangle and nasal tip distance in the two groups were significantly improved compared with those before operation（P〈0.05）, and there was no significant difference in the improvement of the four aboved-mentioned indexes between the two groups（P〈0.05）. There were no significant differences between the two groups in terms of aesthetic compliance rates of nose tip height, nasorostralangle, nasal tip distance, nasal projection and nasolabial angle（P〈0.05）. There were no statistical differences in doctor satisfaction, patient satisfaction and third party satisfaction between the two groups（P〈0.05）. Within 1 year after operation, the incidence rate of complications in autologous group was significantly lower than that in control group（0.00% vs 15.00%）（P〈0.05）. Conclusions The plastic effect of autologous nasal septal cartilage and ear cartilage is similar to bulk filling for nasal tip plasty, but the former one has fewer complications and higher safety.
