两种型全口义齿对老年无牙颌患者颞下颌关节紊乱病影响的比较

Comparison of the effects of two kinds of complete dentures on temporomandibular disorders in elderly patients with edentulous jaws

目的分析比较两种型全口义齿对老年无牙颌患者颞下颌关节紊乱病影响。方法随机选择2016年6月至2018年6月在我科接受全口义齿修复的48例老年无牙颌合并颞下颌关节紊乱病患者,其中有24例患者接受舌侧集中型全口义齿修复(观察组),有24例患者接受解剖型全口义齿修复(对照组),观察两组患者修复效果。结果治疗后,观察组颞下颌关节功能障碍指数改善情况明显优于对照组,组间差异具有统计学意义(P<0.05)。观察组的治疗满意度显著高于对照组,组间差异显著(P<0.05)。结论与解剖型全口义齿修复相比较,舌侧集中型全口义齿修复对老年无牙颌患者颞下颌关节紊乱病的影响治疗效果更优越,能够有效改善患者的临床症状,帮助患者恢复咀嚼功能,减轻患者在义齿适应阶段的负面情绪,以及对颞下颌关节紊乱病的消极心理,有助于改善患者的生活质量,对于老年无牙颌合并颞下颌关节紊乱病患者而言是较为理想的义齿修复方案。

Objective To compare the effects of two kinds of complete dentures on temporomandibular disorders in elderly patients with edentulous jaws. Methods 48 cases of geriatric anodonti with temporomandibular joint disorder（TMJ） were randomly selected from June 2016 to June 2018 in our department. Of them, 24 patients received lingual concentrated complete denture repair（observation group）, and 24 patients received anatomic complete denture repair（control group）, and two were observed. The effect of the patients was repaired. Results After treatment, the improvement of temporomandibular joint dysfunction index in the observation group was better than that in the control group, and the difference was statistically significant（P〈0.05）. Conclusion compared with the repair of complete denture, the restoration of the tongue side concentrated complete denture is more effective in the treatment of the TMJ in the elderly. It can effectively improve the clinical symptoms, help the patients recover the masticatory function, and reduce the negative emotion in the patient＇s denture adaptation stage. And the negative psychology of temporomandibular joint disorder is helpful to improve the quality of life of the patients. It is an ideal denture repair for the elderly patients with edentulous jaws with TMD.
